GOAL/OUTCOME. CONTRACTOR shall: Ensure that a minimum of seventy-five percent (75%) of referrals received by CONTRACTORCase Conferences are processedcompleted within thirty (30) calendar days. Processing includes, but is not limited to, Case Conferences, Client Assessments, and Medical Record Reviews. of the EAFC referral. Conduct Medical Record Reviews in accordance with Subparagraph 2.7 of this Exhibit A of this Agreement and submit a report in a format approved by ADMINISTRATOR, that contains, but is not limited to, summaries of medical record reviews and home visits, as well as reports of capacity assessments and outcomes, and also recommendations for follow-up, to the referring agency within forty-five (45) business days of referral. CONTRACTOR and ADMINISTRATOR may mutually agree, on a case-by-case basis, to extend the report deadline. Ensure that a minimum of seventy-five percent (75%) of Client Assessments are completed within thirty (30) calendar days of the date the assessment is referred to the EAFC. CONTRACTOR and ADMINISTRATOR may mutually agree to extend this deadline. Ensure that a minimum of seventy-five percent (75%) of Capacity Assessments are completed within thirty (30) calendar days of the date the case is presented to the EAFC. CONTRACTOR and ADMINISTRATOR may mutually agree to extend the report deadline. Conduct an annual outcome assessment approved by ADMINISTRATOR, and provide COUNTY with data and statistics specific to services described in this Agreement on a mutually agreed upon basis.

  • STATEWIDE ACHIEVEMENT TESTING When CONTRACTOR is a NPS, per implementation of Senate Bill 484, CONTRACTOR shall administer all Statewide assessments within the California Assessment of Student Performance and Progress (“CAASPP”), Desired Results Developmental Profile (“DRDP”), California Alternative Assessment (“CAA”), achievement and abilities tests (using LEA-authorized assessment instruments), the Fitness Gram, , the English Language Proficiency Assessments for California (“ELPAC”), and as appropriate to the student, and mandated by LEA pursuant to LEA and state and federal guidelines. CONTRACTOR is subject to the alternative accountability system developed pursuant to Education Code section 52052, in the same manner as public schools. Each LEA student placed with CONTRACTOR by the LEA shall be tested by qualified staff of CONTRACTOR in accordance with that accountability program. XXX shall provide test administration training to CONTRACTOR’S qualified staff. CONTRACTOR shall attend LEA test training and comply with completion of all coding requirements as required by XXX.
