Grievance Information. CONTRACTOR shall advise applicants who are denied CONTRACTOR’s services, and recipients who do receive services, of their right to present grievances, and of their right to a State hearing concerning services received under this Agreement.
Grievance Information. Official grievances filed by any teacher under the grievance procedure shall not be placed in the personnel file of that teacher.
Grievance Information. All written grievances must contain the following information to be considered:
A. Name of xxxxxxxx;
B. Grievant's rank, if applicable;
X. Xxxx of incident giving rise to grievance;
D. Date grievance is being filed in writing;
E. Name of supervisor with whom xxxxxxxxx was discussed, and date of discussion, if applicable;
F. Description of incident giving rise to the grievance;
G. Articles and Sections of Agreement violated;
H. Resolution requested. The Lodge shall develop a grievance form which shall provide the information outlined in this Section. The Lodge shall have the responsibility for duplication, distribution and its own accounting of the grievance forms.
Grievance Information. Upon the request of the individual employee, public information pertinent to their particular grievance will be made available by the District. Additional costs that result from the request for information will be paid by the employee or the Association.
Grievance Information. All grievances must contain the following information to be considered and must be filed using the grievance form mutually agreed upon by both parties:
1. Aggrieved employee’s name and signature.
2. Aggrieved employee’s classification.
3. Xxxx xxxxxxxxx was first discussed and name of the officer with whom the grievance was discussed.
4. Date grievance was filed in writing.
5. Date and time grievance occurred.
6. The location where the grievance occurred.
7. A description of the incident(s) giving rise to the grievance.
8. Specific Articles and Sections of the Agreement violated.
9. Xxxxxxx remedy to resolve the grievance.

Grievance Information. Upon request by an authorized member of the Association, the University shall provide the Association with all information required for the preparation and processing of a grievance, as specified in the grievance procedure.
Grievance Information. A grievance shall mean an allegation by a Principal, or the St. Xxxxxxx-Albertville Principals' Association on behalf of a Principal, resulting in a disagreement regarding the interpretation or application of terms and conditions of employment insofar as such matters are contained in this Agreement. Pursuant to M.S. §179A.20, subd. 4, a disciplinary action shall constitute a grievance which may be submitted to the grievance procedure.
Grievance Information. The College Administration agrees to furnish the Chairman of the Grievance Committee, upon timely request, available information concerning the following: annual financial reports, register of professional personnel, minutes of all Board meetings, students' census and Faculty membership data, Faculty rank, and will provide the Federation with available information necessary to process any grievance of the Contract.
