Common use of HEALTH AND WELFARE PLAN Clause in Contracts

HEALTH AND WELFARE PLAN. 17.01 The Employer agrees to pay the amount as set out in Schedules “A” for all hours worked for each employee towards the Insurance Plan administered by the CLAC Health and Welfare Trust Fund. 17.02 a) Employees are eligible to receive coverage according to the rules of the Trust Fund. It is the responsibility of the employee to complete the enrolment form for the benefit plan, which is a condition of coverage.
