HEALTH & WELFARE. (a) The Employer agrees to pay 100% of the Ontario Health Tax for all eligible nurses. (b) The Employer agrees to contribute seventy-five percent (75%) of the billed premiums under the Blue Cross Dental Plan #9 (or equivalent) based on the current ODA schedule as it may be updated from time to time. Major Dental 50/50 co-insurance which shall include complete and partial dentures, Crowns, bridgework, and repairs to same and inlays and onlays and orthodontics two thousand ($2000) per insured dependent child. (c) Group Life Insurance providing coverage in the amount of twice the annual salary of the full-time nurse and in the principal amount equal to the amount of the Group Life Insurance to which the nurse is entitled, paid at 100% by the Employer. (d) The Employer agrees to contribute seventy-five percent (75%) of the billed premiums toward coverage of the eligible nurse in the active employ of the Employer under a Metropolitan Insurance extended Health Care plan with no deductible. In addition to the standard benefit coverage this will include hearing aids (maximum $300.00/person), vision care ($400.00/24 months), one eye exam per insured person every 24 months and chiropractic, massage therapy, physiotherapy, naturopath and podiatrist (maximum for each of the foregoing of $350 per insured person annually). Coverage for mental health services by a Psychologist, Registered Psychotherapist or Social Worker (MSW) for a total of $500 annually. (e) The Extended Health Care benefits shall include coverage for Diabetic blood testing supplies and equipment. The Employer will provide each nurse with a copy of the Extended Health Benefits brochure 22.02 Any dispute which may arise concerning a nurse's entitlement to benefits may be subject to grievance and arbitration under the provisions of this Agreement. The Employer may substitute another carrier for any of the foregoing plans (other than OHIP) provided that the level of benefits conferred thereby are not decreased. The Employer will advise the Association of any change in carrier or underwriter at least sixty (60) days prior to implementing a change in carrier. The Employer will provide the Association with a summary document outlining the differences, if any, between the levels of benefits provided by the existing and new carrier plans.

HEALTH & WELFARE. 17.01 All full-time employees covered by the Collective Agreement, upon completion of their probationary period, will be covered by the CLS Services Ltd group insurance plan for hourly employees. Coverage includes, Extended Healthcare (ato include a drug card), Dental, Basic Life Insurance and Short-term disability benefits. The Company may change benefit carriers at its dis- cretion provided the Union is notified in writing and the coverage is equivalent to current coverage, however, the following shall apply: Extended Healthcare deductible - fifty dollars ($50.00) per calendar year. Paramedical Services - payable up to three hundred and fifty dollars ($350.00) per service, per calendar year Orthotics are payable up to two hundred dollars ($200.00) per calendar year. Drug Plan – Dispensing fee cap of eight dollars ($8). Vision Care Allowance – Two hundred dollars ($200) every two (2) years. 17.02 The Employer agrees Employer’s sole responsibility shall be to pay 100% the Employer’s portion of the Ontario Health Tax premium for all eligible nursesthe plan. The premium is 85% Employer paid and 15% Employee paid. (b) 17.03 The parties agree that benefit issues are not arbitrable under the terms of the Collective Agreement. The Company agrees that the Union shall be entitled to represent their members in consultation with the Insurance Carrier in the event of any disputes between the employee and the Insurance Carrier. 17.04 Should an employee become disabled as a result of an occupational accident or illness, the Employer agrees to contribute seventy-five percent one hundred (75100%) percent of the billed premiums under required contributions for the Blue Cross Dental Plan #9 (or equivalent) based on period of disability, provided the current ODA schedule em- ployee remits their portion of the premium as it may be updated from time to time. Sick days shall not be carried over from one year to the next. Effective 1 January 2016 employees shall be eligible to receive six (6) sick days Each full-time employee with at least one year of contin- uous service shall accrue sick days based on one-half (1/2) day per month to a nurse's entitlement maximum of six (6) days per year. Sick days shall not be carried over from one year to the next. 17.07 Accrued sick leave credits will be reduced when an em- ployee is absent due to illness or injury until such time as the credits are exhausted or disability insurance benefits commences following the applicable waiting period. Employees may be subject required to grievance and arbitration under bring in a doctor’s note for public health purposes or when absent for three (3) days or more upon their return to work, in which case the provisions cost of this Agreementobtaining the medical certificate shall be xxxx- bursed by the Company to a maximum of twenty dollars ($20.00). The Employer Company may substitute another carrier also require a medical cert- ificate in circumstances which tend to indicate patterns of absence or repetitive ailments in which case, the em- ployee shall be responsible for any the cost of the foregoing plans (other than OHIP) provided that the level of benefits conferred thereby are not decreased. The Employer will advise the Association of any change in carrier or underwriter at least sixty (60) days prior to implementing a change in carrier. The Employer will provide the Association with a summary document outlining the differences, if any, between the levels of benefits provided by the existing and new carrier planssuch note.

HEALTH & WELFARE. (a19.01 An employee who chooses to opt out of any Health and Welfare benefits outlines in this Article, shall be entitled to enroll in the benefits under any one of the following conditions: i) A life changing event, such as divorce or death of a spouse; ii) When an employee transfers from a part time classification to a full time classification NOTE: It shall be the joint responsibility of the Employer and employee to ensure that if the employee wishes to participate she signs the appropriate enrolment documents in a timely fashion. Employees who opt out of benefits will do so in writing on a form provided by the Employer. 19.02 The Employer agrees to pay the indicated percentages of the following items for fulltime Employees who have completed three (3) calendar months of employment and who qualify under the terms of the plans and who subscribe to said plans. (i) Ontario Health Insurance Plan or equivalent: 100% of the Ontario Health Tax for all eligible nursesbilled rate of premium. (bii) The 100% of the billed rate of premium of the Group Plan, providing $25,000 term life. (iii) An Extended Health Care Plan will be available to Employees and the Employer agrees shall pay 100% of the billed rate of a $10/$20 deductible plan. Within the Extended Health Care Plan there shall be provision for eye glasses to contribute seventy-five a maximum of $160.00 per person in each consecutive two (2) year period and hearing aids prescribed by an Orolargyagologist to a maximum of $300.00 during the lifetime of each insured person. It is' the responsibility 'of the Employee to submit claims. Employees must complete either an application for enrolment or a summary document outlining the differences, if any, between the levels written refusal of benefits provided by the existing and new carrier plansall coverage within thirty (30) days of completion of probation.
