1. Instructors authorized to work for a minimum of thirty-five (35) hours per week will receive fully paid health insurance provided other regular teachers.

The Board shall make payment of insurance premiums for each employee, providing they have made proper application, to provide insurance coverage for the full time twelve- month period commencing September 1 and ending August 31. When necessary, premiums in behalf of the teacher shall receive a prorated amount toward MESSA ABC I OR MESSA ABC 2 be made retroactively or prospectively to assure uninterrupted participation and coverage. The District shall, immediately upon receipt of the benefit from the insurance carrier, issue a check to the bargaining unit member in the full amount of the original benefit, less applicable taxes. The school district shall report the appropriate percentage of pro‐rated time based upon the percentage of benefit received to the Michigan Public School Employees Retirement System (MPSERS) for credit. Example: HEALTHA member on LTD at a 66 2/3% benefit for 180 days would have 120 days (66 2/3% of 180) credited for retirement purposes with MPSERS. It is understood and agreed that this provision is in accordance with recent rulings of MPSERS to allow such credit. Health insurance will be provided to high school completion and summer school instructors based upon the following schedule:
1. Instructors authorized to work for a minimum of thirty-five (35) hours per week will receive fully paid health insurance provided other regular teachers.
2. Instructors authorized to work for a minimum of twenty (20) hours each week but less than thirty-five (35) hours per week will receive one-half (1/2) of the monthly health insurance premium paid by the Board provided the teacher authorizes payroll deduction for the remainder of the premium cost.
3. Summer school instructors, if eligible, will receive health insurance benefits as provided in 1 and 2 above only during the actual months of employment. The employee will be provided the above listed insurance coverage if he/she is not covered elsewhere by a health care protection plan.
B. In the event that an employee, absent because of illness or injury, has exhausted sick leave accrual the above mentioned fringe benefits shall continue throughout the balance of the school year.
C. The Board shall make payment of insurance premiums for each employee, providing they have made proper application, to provide insurance coverage for the full time twelve- month period commencing September 1 and ending August 31. When necessary, premiums in behalf of the teacher shall receive a prorated amount toward MESSA ABC I OR MESSA ABC 2 be made retroactively or prospectively to assure uninterrupted participation and coverage. C. The parties agree to the following insurance enrollment procedures:
1. New employees will be enrolled effective their first day of $ 50,000 with A D & D VISION – VSP 3+ DENTAL – 80/80/80/: $ 2, 000 with Adult Ortho Persons choosing PAK B shall receive a cash option in lieu of health benefitswork. The Board district agrees to forward the enrollment applications as soon as they have been completed by each new employee and after the employee has worked one (1) day for the district.
2. Beginning with the 1996-97 school year, September 1st shall develop and implement a qualified plan document which complies with Section 125 of be the Internal Revenue Codeeffective date for the district to pick up insurance coverage for new employees.
3. The amount of open enrollment period for the cash payment bargaining unit shall be $4420.00 yearly and may be applied to a Tax-Deferred Annuity. To elect the annuity, the member shall enter into a salary reduction agreementmonth of September.

B. When appropriate, Medicare premiums will be paid on behalf of the teacher, eligible spouse, or dependent until the date of the teacher's retirement.
C. In the event that an employee, absent because of illness or injury not connected with the discharge of his duties, has exhausted his sick leave accrual, the insurance coverages provided in the Article shall continue to the benefit of said teacher throughout the balance of the contract year. In the event such illness or injury is contracted or incurred in the discharge of the teacher's duties, the insurance coverages provided in this Article shall continue to the benefit of said teacher for the full duration of such illness or injury.
D. The Board shall make payment of insurance premiums for each employee to provide insurance coverage for the full twelve-month period commencing September 1 and implement ending August 31 of each year for all teachers who complete their contractual obligations. If a teacher terminates his or her employment for reasons other than illness or Family Care leave prior to June, said teacher's subsidy shall terminate on the first of the month following. When necessary, premiums on behalf of the teacher shall be paid retroactively or prospectively to ensure uninterrupted participation and coverage, when authorized by the teacher in writing. The Superintendent shall notify the teacher when such authorization is required. Provisions for repayment to the Board when necessary shall be a part of the written authorization.
E. In the event any teacher is laid off under the provisions of Article X of the Agreement, the insurance coverages provided in this Article shall continue to the benefit of said teacher for a period of one year from the date of layoff or until said teacher attains a minimum of full-family health insurance coverage from another employer, whichever period of time shall be the lesser. This section is contingent upon approval of coverage by the respective insurance carriers.
F. During the term of the Agreement, representatives of the Board and Association will meet to investigate improvements in insurance coverage provided under this article. It is understood that the changes in benefit patterns and/or carriers which may result from such investigations may be made only by mutual agreement and without the necessity of reopening this Agreement for renegotiation.
G. The insurance protection provided in this Article shall remain in effect until a successor to this Agreement is ratified and signed.
H. The Board shall formally adopt a qualified plan document which complies with Section 125 of the Internal Revenue Code. C. Employees not wishing to have the health insurance may elect at a maximum cost to the Board of $240.00 per month, any of the MESSA and/or MEFSA options. If a husband and wife are both members of this Bargaining Unit, one shall elect Plan A total of $ 50,000 with A D & D VISION – VSP 3Plus DENTAL – 80/80/80: $ 2,000 with Adult Ortho PAK B: LONG TERM DISABILITY – same as above NEGOTAITED LIFE – A total of $ 50,000 with A D & D VISION – VSP 3+ DENTAL – 80/80/80/: $ 2and other shall elect options.
D. Teachers assigned less than full time but at least one-half time, 000 with Adult Ortho Persons choosing PAK B shall receive Plan B without cost to the Bargaining Unit Member, or Plan A on a pro-rata basis, based on Plan A costs for that member. Teachers assigned less than one-half of a full schedule shall receive the benefits in Plan B, or Plan A on a pro-rata basis based on Plan A costs for that member.
E. The Board shall make payment of insurance premiums for all persons who complete their contractual obligation to assure insurance coverage for the full twelve-month period commencing September 1st and ending August 31st, even though the teacher may not be returning to the next school year. The open enrollment period shall be established jointly by the Board, the Association, and insurance company representative, including opportunities for summer pre- enrollment and fall open-enrollment as well as whenever group or individual subsidy amounts increase or decrease, affecting the benefit package. The employee shall contribute 3% of salary for PAK A and Choices II. In the event that a teacher is terminated or resigns during the school year, the insurance shall be continued until the teacher has received the pro-rata portion of the twelve-month insurance year earned at the time of the termination or resignation.
F. A teacher who is hired with an effective first work day after the first required work day of the school year shall be entitled to fringe benefits for a duration determined on a pro-rata basis.
G. When necessary, premiums on behalf of the teachers shall be made retroactively or prospectively to assure uninterrupted participation and coverage.
H. The Board will be responsible for providing insurance information including application, claim materials, and enrollment meetings for the above mentioned programs.
I. Payroll deductions shall be available for all MESSA, MEFSA, and MEA programs.
1. The employer shall provide a cash option in lieu of electing the health benefitsbenefit. The Board cash amount shall develop and implement be $240.00. The employer shall formally adopt a qualified plan document which complies with pursuant to Section 125 of the Internal Revenue CodeService code. INSURANCE PROTECTION. The 21.1 Pursuant to the authority, as set forth in the Michigan School Code Section 380.632, the Board shall agrees to provide twelvea hospitalization insurance fringe benefit.
21.2 Effective November 1, 1989, and upon acceptance of a written application, the Board of Education will provide each full-month coverage for all bargaining unit members through XXXXX-XXX. 3. Effective July 1, 2011 Board monthly premium contribution will be limited to: Single Subscriber $ 747.39 2-Person $1,501.09 Family $1,651.84 Effective July 1, 2012, the Board of Education shall pay any increase in premiums for health care coverage through August 31, 2012. Any premium contribution by the Alcona EA members then in effect, shall remain in effect. B. Part-time bargaining unit members may participate in either Plan A or Plan B, provided said part-time employee first executes a payroll deduction for the employee's pro rata contribution towards the cost of $ 50,000 either Plan A or Plan B. The Board shall make premium contributions for part-time teachers pro-rated on the basis of the number of workdays and hours for a full-time, regularly employed teacher. Benefits for part-time teachers in job-sharing arrangements are addressed separately in Article 18 of this Agreement.
C. The Board will not contribute toward the cost of a medical insurance (health, dental, vision, LTD, life) for a teacher on unpaid leave of absence, unless otherwise required by the Family and Medical Leave Act of 1993.
D. The Board will continue to pay insurance premium contributions through June 30 of the year in which a teacher retires for those teachers completing the school year. For those teachers retiring other than at the end of the school year, the Board shall contribute premiums for coverage to and including the last date of the calendar month in which the teacher's separation becomes effective.
E. Except as otherwise provided above, the Board will continue to pay insurance premium contributions through August for teachers laid off or otherwise leaving the system at the close of the school year.
F. The Board will continue to pay insurance premium contributions through June for teachers who enter the system after the beginning of the second semester and remain for the balance of the school year.
G. The Board will not continue to contribute toward the cost of insurance for a teacher whose employment terminates before the close of the school year.
H. Newly hired employees shall be enrolled for coverage within thirty (30) days of commencing work, provided the insurance carriers, policyholders and plan administrators so allow.
I. Dual coverage (i.e. internal coordination of benefits) for both a husband and wife (as well as dependent children) shall not be permitted where both spouses are employed by the Xxxxx Rapids Public Schools and are otherwise eligible for coverage. In that event, one spouse shall be designated as a dependent for insurance coverage purposes. That person shall instead be enrolled in Plan B, above. This limitation shall also apply in situations where the employees are ex-spouses with A D & D VISION – VSP 3Plus DENTAL – 80/80/80: $ 2,000 regard to coverage for their dependent children.
J. B. When appropriate, Medicare premiums will be paid on behalf of the teacher, eligible spouse, or dependent until the date of the teacher's retirement.
C. In the event that an employee, absent because of illness or injury not connected with the discharge of his duties, has exhausted his sick leave accrual, the insurance coverages provided in the Article shall continue to the benefit of said teacher throughout the balance of the contract year. In the event such illness or injury is contracted or incurred in the discharge of the teacher's duties, the insurance coverages provided in this Article shall continue to the benefit of said teacher for the full duration of such illness or injury.
D. The Board shall make payment of insurance premiums for each employee to provide insurance coverage for the full twelve-month period commencing September 1 and implement ending August 31 of each year for all teachers who complete their contractual obligations. If a teacher terminates his or her employment for reasons other than illness or Family Care leave prior to June, said teacher's subsidy shall terminate on the first of the month following. When necessary, premiums on behalf of the teacher shall be paid retroactively or prospectively to ensure uninterrupted participation and coverage, when authorized by the teacher in writing. The Superintendent shall notify the teacher when such authorization is required. Provisions for repayment to the Board when necessary shall be a part of the written authorization.
E. In the event any teacher is laid off under the provisions of Article X of the Agreement, the insurance coverages provided in this Article shall continue to the benefit of said teacher for a period of one year from the date of layoff or until said teacher attains a minimum of full-family health insurance coverage from another employer, whichever period of time shall be the lesser. This section is contingent upon approval of coverage by the respective insurance carriers.
F. During the term of the Agreement, representatives of the Board and Association will meet to investigate improvements in insurance coverage provided under this article. It is understood that the changes in benefit patterns and/or carriers which may result from such investigations may be made only by mutual agreement and without the necessity of reopening this Agreement for renegotiation.
G. The insurance protection provided in this Article shall remain in effect until a successor to this Agreement is ratified and signed.
H. The Board shall formally adopt a qualified plan document which complies with Section 125 of the Internal Revenue Code. The amount of the cash payment shall be $4420.00 yearly and may be applied to a Tax-Deferred Annuity. To elect the annuity, the member shall enter into a salary reduction agreement. The Board and the employee shall be responsible for paying their respective FICA taxes on the cash payment. Costs relating to the implementation and administration of benefits under this program shall be borne by the Board. INSURANCE PROTECTION. A. The Board will provide, without cost to the bargaining unit member, bodily injury and property damage insurance limited to a maximum of one million dollars ($1,000,000.00).
B. Upon proper application and acceptance for enrollment by the appropriate insurance underwriter, the Board shall make premium payments on behalf of full-time bargaining unit members and their eligible dependents under either Plan A or Plan B as specified below for the duration of this Agreement.
