Life Plans. The employee’s domestic partner and eligible dependent(s) may elect coverage through the voluntary life program. The voluntary life program will be administered under the provisions found in the insurance brochure. Life plans and existing coverage are subject to change.

Life Plans. The employee’s same sex spouse/domestic partner and eligible dependent(s) may elect coverage through the supplemental life insurance program. Contribution Schedules The Company will pay the same percentage share of the premium costs for the coverage of an employee with a same sex spouse/domestic part- ner, or same sex spouse/domestic partner with dependent(s), as it pays toward the cost of coverage of an employee with an opposite sex spouse or opposite sex spouse with dependent(s). The employee will be subject to the annual schedule of premium con- tributions for employee, employee plus child(ren), employee plus spouse, and employee plus family coverage. The scheduled premium contribution for the same sex spouse/domestic partner’s and/or same sex spouse/do- mestic partner's dependent child(ren)'s benefits will be deducted from the employee’s payroll checks on an after-tax basis.

  • Retirement Plans In connection with the individual retirement accounts, simplified employee pension plans, rollover individual retirement plans, educational IRAs and XXXX individual retirement accounts (“XXX Plans”), 403(b) Plans and money purchase and profit sharing plans (collectively, the “Retirement Plans”) within the meaning of Section 408 of the Internal Revenue Code of 1986, as amended (the “Code”) sponsored by a Fund for which contributions of the Fund’s shareholders (the “Participants”) are invested solely in Shares of the Fund, JHSS shall provide the following administrative services:

  • Meal Plans Residents living in Residence Facility are required to purchase a University meal plan. Information regarding the meal plan options can be obtained by contacting the meal plan office at 000-000-0000.

  • Dental Plans A. The dental plans offered and benefits provided by those plans shall be determined by the Personnel Department in accordance with LAAC Section 4.303, upon the recommendation of the JLMBC and approval of the City Council.

  • Insurance Plans The Executive is eligible to participate in the life, health, dental, short and long-term disability plans made available to the employees of the Company pursuant to the terms and conditions of such plans.

  • Medical Plans The Employer will maintain the current health (including vision) and dental insurance programs and practices. For Calendar Years 2022 — 2023, the Employer shall contribute 80% of the premium charge for PPO plans, 85% of premium for the EPO plan, 85% of premium for the IHM plan, 80% for the prescription drug plan and 50% for the dental plan.

  • Retirees The Parties and the Crown agree to meet for the purpose of transitioning retirees currently in board-run benefits plans into a segregated plan administered by the OECTA ELHT via an amendment to the Trust Agreement, based on the following:

  • GROUP INSURANCE PLANS 15.01 The Company agrees to provide all full time employees with one (1) or more years of continuous service, a *Short Term Disability Benefit (S.T.D.) as generally described in Section B of a notice to all employees of the amended Benefit Plan dated May 1, 1993. *Payment for Short Term Disability shall begin on the third (3rd) consecutive day of absence. For those employees who have completed ninety (90) days of perfect attendance from the last date of return to work from an absence due to sickness or accident, shall be paid S.T.D. from the first (1st) day for the first covered absence following the ninety (90) days of perfect attendance.
