Mandatory Reporting Requirements. The Provider and any subcontractor must comply with and inform its employees of the following mandatory reporting requirements. Each employee of the Provider, and of any subcontractor, providing services in connection with this Contract who has any knowledge of a reportable incident shall report such incident as follows: 1) reportable incidents that may involve an immediate or impending impact on the health or safety of a client shall be immediately reported to the Contract Manager; and 2) other reportable incidents shall be reported to the Department’s Office of Inspector General through the Internet at xxxx://xxx.xxx.xxxxx.xx.xx/admin/ig/rptfraud1.shtml or by completing a Notification/Investigation Request (Form CF 1934) and emailing the request to the Office of Inspector General at XX.Xxxxxxxxxx@xxxxxxxxxxxx.xxx. A reportable incident is defined in Children and Families Operating Procedure (CFOP) 180-4, which can be obtained from the Contract Manager.
Mandatory Reporting Requirements a. All employees, subcontractors and volunteers of Contractor who provide a program, activity, or service to the University that are responsible for the care, supervision, guidance, or control of children are considered mandated reporters of suspected cases of child abuse under Pennsylvania law. All mandated reporters shall make an immediate report of suspected child abuse if the individual has reasonable cause to suspect that a child is the victim of child abuse under any of the following circumstances: (i)the mandated reporter comes into contact with the child in the course of employment, occupation and practice of a profession or through a regularly scheduled program, activity or service; (ii) the mandated reporter is directly responsible for the care, supervision, guidance or training of the child, or is affiliated with an agency, institution, organization, school, regularly established church or religious organization or other entity that is directly responsible for the care, supervision, guidance or training of the child; (iii) a person makes a specific disclosure to the mandated reporter that an identifiable child is the victim of child abuse; or (iv) an individual 14 years of age or older makes a specific disclosure to the mandated reporter that the individual has committed child abuse. The minor is not required to come before the mandated reporter in order for the mandated reporter to make a report of suspected child abuse. The mandated reporter does not need to determine the identity of the person responsible for the child abuse to make a report of suspected child abuse.
b. Mandated reporters must immediately make an oral report of suspected child abuse to the Department of Human Services (DHS), formerly the Department of Public Welfare, by calling ChildLine at 0-000-000-0000, or by filing a written report with DHS through Pennsylvania’s Child Welfare Information Solution portal (xxxxx://xxx.xxxxxxx.xxxxx.xx.

Each employee of the Lead Agency, and of any subcontractor, providing services in connection with this Contract who has any knowledge of a reportable incident (as defined in CFOP 180-4) shall report such incident as follows: 1) reportable incidents that may involve an immediate or impending impact on the health or safety of a client shall be immediately reported to the contract manager; and 2) other reportable incidents shall be reported to the Department’s Office of Inspector General by completing a Notification/Investigation Request (Form CF 1934) and emailing the request to the Office of Inspector General at xx_xxxxxxxxxx@xxxxxxxxxxxx.xxx. In accordance with the Department’s added mandatory reporting requirements outlined in CFOP 180-4, Network Provider must comply with and inform its employees of the following mandatory reporting requirements which include incidents of fraud, waste, and abuse. Each employee of the Network Provider in connection with this contact who has any knowledge of a reportable incident shall report such incident as follows:
(a) reportable incidents that may involve an immediate or impending impact on the health or safety of client shall be immediately reported to the ChildNet Contract Manager; and
(b) other reportable incidents shall be reported to the Department’s Office of Inspector General through the Internet at xxxx://xxx.xxxxxxxxxxxx.xxx/about-us/office- inspector-general or by completing a Notification/Investigation Request (from CF 1934) and emailing the request to the Office of Inspector General at XX.Xxxxxxxxxx@xxxxxxxxxxxx.xxx. (c) a reportable incident is defined in CFOP 180-4 as follows: i Inappropriate employee acts or omissions that result in client injury, abuse, neglect or death; ii Fraud; iii Theft; iv Breaches of confidentiality, unless inadvertent and self-reported (e.g., revealing a reporter’s name, providing confidential documents to unauthorized persons, access of client files for non-business reasons, providing information from client files such as medical or benefits information, etc.) immediately upon confirmation by the circuit/region:

Mandatory Reporting Requirements. All Broker generated referrals, whether sent to a Prudential Real Estate affiliate, another PRS Member or non-PRS broker, all referrals received and information as to referrals converted shall be reported in accordance with the PRS Policy Manual. This reporting is mandatory.

Mandatory Reporting Requirements. Teachers are required by law to report suspicions of child abuse/neglect to the appropriate agency.
