Benefits Information. (a) The Hospital shall provide each employee with access to information booklets outlining all of the current provisions in the benefit plans defined in article 20.01. Upon request, the Hospital will make the Plan(s) available to the Union for inspection.
(b) The Hospital shall notify the Union of the name(s) of the carrier(s) which provide the benefit plans defined in Article
20.01. The Hospital shall also provide the Union with access to all current information booklets provided to the employees.
Benefits Information. Upon request, the Employer agrees to provide the Union with any statistical and other relevant information at its disposal pertaining to the Pension Plan, Group Life Insurance and Disability Insurance. Information pertaining to benefits for employees can be found on the Human Resources website.
Benefits Information. Laid off employees who wish to remove themselves from the central recall pool will submit a written request to the Human Resources Department who will notify the Committee established in Section 2 below. When a position becomes available, the position shall be offered to the person with the greatest bargaining unit seniority in the job classification who has filed a master application and applied for the job unless said person is not capable of performing the available work.

Benefits Information. CAMH shall provide each employee with access to information booklets outlining all of the current provisions in the benefit plans offered to the employees. Upon request, CAMH will make the plan(s) available to the Union for inspection. CAMH shall notify the Union of the name(s) of the carrier(s) which provides the benefit plans offered to employees. CAMH shall also provide the Union with access to all current information booklets provided to the employees.
Benefits Information. Customer shall provide all Plan information, open enrollment materials and TPA key contacts;

The employer will supply the Union with: • Plan details for weekly indemnity, LTD, Life Insur- ance, Health and Dental Benefits and Pension Plans • Copies of hours report to CWIPP/SIN (monthly) and termination notices • Physical Demands Analysis Reports, if available • WSIB Injury Summary Reports Bereavement days, Union business, jury duty, designated in- xxxxxxx weather shifts or days and all other approved leaves of absence will not be considered as Emergency days.

Effective May Hospital and Extended Health Care benefits will be extended to active full-time employees from the age of sixty-five and up to the employee’s seventieth birthday, on the same cost share basis as those employees under the age of sixty-five (65).
Benefits Information a. The Board shall provide each teacher with an application or enrolment form for participation in the medical, dental, extended health and group life insurance benefit plans. In the event a teacher does not wish to participate in any particular benefit plan where opting out is an option, the application or enrolment form must be so noted by the teacher and kept on file by the Board.
b. The Board shall advise each teacher by the end of September, and all teachers hired subsequent to that date at the end of the first month of employment, of those benefit plans available to teachers, the cost of those plans, and of those plans in which the teacher is enrolled.
c. The Board shall assist teachers in obtaining required benefits from the various benefit plans.
d. The Board shall advise all eligible teachers that they must contribute to the Teachers' Pension Plan.
e. All teachers are entitled to participation in all benefit plans save and except:
i. Teachers Teaching on Call; and
ii. part-time teachers pursuant to Article B.24. (Part-Time Teachers)
f. Teachers Teaching on Call may participate in the Provincial Extended Health Benefit Plan by paying 100% of the premium costs.
g. The Board shall advise each teacher in writing at the end of September, December and March of their accumulated sick leave.

Upon request, the Hospital will make the official Health and Welfare Benefit plan(s) documents available to the Union for inspection.
(b) The Hospital shall notify the Union of the name(s) of the carrier(s) which provide the Health and Welfare Benefit plans defined in Article 39.
01. The Hospital shall also provide the Union with access to all current information booklets provided to the employees.
(c) In the event of a conflict between the terms of the official Health and Welfare Benefit plan(s) documents and the information booklets provided to employees, the terms of the official plan documents will govern.
