Reportable Incidents. Environmental Compliance
Reportable Incidents. (a) The Contractor must report any Reportable Incident to AAQ on the date it occurs, or on the following Business Day if the Reportable Incident occurred outside of Business Hours. Reportable Incidents include any improper conduct in relation to a Consumer, AAQ, the Contractor, Personnel or otherwise in relation to the Services.
(b) AAQ will inform the Contractor of any allegations of improper conduct which involve the Contractor or its Personnel, including where such allegations are received from a third party.
(c) Where there has been a Reportable Incident, AAQ may take all necessary action in relation to a Reportable Incident, to ensure the immediate safety and welfare of the Consumer. The Contractor agrees to cooperate, and ensure that its Personnel cooperate, with such necessary action.
Reportable Incidents. (a) In the event of a Reportable Incident, the Housing Support Provider must:
(i) prioritise the health and wellbeing of those directly affected in the Reportable Incident;
(ii) restore a safe environment as soon as possible following the Reportable Incident;
(iii) minimise the opportunity for others to be exposed to the Reportable Incident environment;
(iv) notify the relevant person with an interest in the Reportable Incident of the Reportable Incident, including next of kin, police, support services as may be appropriate;
(v) notify the Owner of the Reportable Incident:
(A) verbally within 24 hours of the Reportable Incident; and
(B) in writing within two Business Days of the Reportable Incident occurring in the form as set out in 'Attachment 8: Reportable Incident Form';
(vi) where appropriate:
(A) actively contribute to any enquiry and/or investigation of the Reportable Incident that may be undertaken by any Government Body, including the Owner;
(B) participate in the implementation of any recommendations of any Government Body, including the Owner, following the Reportable Incident;
(C) manage verbal and documented information according to any legal obligations.
(b) In this clause 15.18, a Reportable Incident means:
(i) an event or circumstance that is unexpected and unintended during the performance by the Housing Support Provider in accordance with this Deed that led to harm or suffering, loss or damage to a Tenant or Household resulting in death, injury or admission to hospital;
(ii) theft or misappropriation of rent or other income derived from the Premises;
(iii) complete, or major, disruption to the performance by the Housing Support Provider of the obligations under this Deed resulting from:
(A) an environmental event (such as fire or flood) requiring evacuation and temporary or permanent closure or significant reduction in service;
(B) a major external review that recommends closure or reduction in service provision or resources; or
(C) financial loss, as a result of theft or misappropriation;
(iv) a Reportable Incident is not an event that is:
(A) expected or usual to the behaviour or ongoing health and wellbeing of a Tenant or Household;
(B) one occurring to the Housing Support Provider’s Agents;
(C) an allegation, which is a claim that has not been tested or substantiated; or
(D) a complaint.
Reportable Incidents. 6.1. If a Reportable Incident occurs involving the vehicle or its operation, the Bailee will;
6.1.1 As soon as practicable, provide a report of the incident to the ▇▇▇▇▇▇, including the following particulars (if known):
a) Any party involved in the incident
b) If the incident involves a collision with another vehicle, the registration details of the other vehicle and the personal details of the driver of that vehicle
c) Names and contact details of any witnesses;
d) Whether any person suffered an apparent injury as a result of the incident; and
e) Any obvious damage to or loss of property arising out of the incident.
6.1.2 Within 48 hours of receiving any below notice, give to the ▇▇▇▇▇▇:
a) A copy of any statement made by the Bailee to the police concerning the incident;
b) The incident record number issued to the Bailee by the Police; and
c) Any document posted, delivered or served on the Bailee by or on behalf of any person or party making any claim against the Bailee and/or ▇▇▇▇▇▇.
Reportable Incidents a. If there is reasonable cause to believe a consumer has been abused, neglected, or financially exploited, the provider must immediately, day or night, contact the 24-hour ELDER ABUSE HOTLINE at 1-800-922-2275.
b. The Provider must report to the ASAP the same business day any hospitalization, addition or loss of a household member, consumer’s absence from the home, alleged theft, alleged breakage of consumer’s possessions, injury to employee or consumer, or consumer complaint.
c. The consumer and ASAP must be notified of a canceled visit or any variation in service delivery from the written authorization.
d. The provider must report to the ASAP by the next business day a new consumer address, name, or telephone number; new MD, new diagnosis, and employee complaints.
Reportable Incidents. Reportable Incident form will be available at LRH and can be completed by LRH residents, LRH visitors/guest and staff. All Reportable Incidents will be investigated by the LRH Director within 24 hours of the report being received.
Reportable Incidents. Within 24 hours, MDOC will notify the United States and the DQE of suicides and all serious suicide attempts (i.e., suicide attempts requiring medical hospital admission). The notification will include the following information:
a. Incident report, name, housing unit location, brief summary or description, mental health classification, security classification, date of birth, date of incarceration, and date of incident.
Reportable Incidents. CONTRACTOR shall report within 24' hours all incidents affecting the immediate health, safety and well being of clients to the office of the Director of Mental Health. Reportable incidents include, but are not limited to, all deaths, episodes of acute life threatening illness, serious physical or psychological injuries (or risk thereof), and allegations of abuse and/or neglect. CONTRACTOR shall establish procedures for the investigation of such incidents and shall cooperate with any additional investigation COUNTY may wish to conduct.
Reportable Incidents. 3.1 Reportable incidents include:
(a) unintended movement of railroad equipment;
(b) failure to provide track protection for Workers when required;
Reportable Incidents. All Guards assigned to this Master Agreement shall write Incident Reports in any of the following circumstances, including but not limited to:
a. Guard is required to make any physical contact with a member or members of the public, City staff or other guards;
b. Guard is required to intervene between any two or more persons including other guards;
c. Guard witnesses any crime or suspected crime, including assault;
d. Guard witnesses any incident in which there is a potential injury whether or not medical attention is immediately required, or in which loss or damage to public or private property occurs;
e. Guard observes hazardous conditions;
f. Guard is required to give direction or order to a tenant or public and the tenant or public protest or express their unwillingness to comply;
g. ▇▇▇▇▇ observes persons attempting to gain unauthorized entry;
h. Guard discovers any unlocked doors or any activated alarms, false or otherwise;
i. ▇▇▇▇▇ discovers any evidence of an area being used and/or occupied by vagrants or loiterers; and
j. ▇▇▇▇▇ observes suspicious or unusual activities.
