An employee who is pregnant and who has three (3) months or more of continuous service with the Company shall be granted leave of absence under the terms and conditions of the Employment Standards Act except as herein otherwise provided: (a) An application for such leave will be submitted on forms supplied by the Company and will include a certificate signed by the employee’s doctor specifying the estimated date of delivery.
