Membership and Services. Member’s membership is a contractual privilege to use the Sports Academy facilities, equipment, and training services, and participate in Sports Academy sponsored events, activities, and programs offered from time to time by the Sports Academy (for an additional fee, as applicable) as applicable to the particular membership options selected, training sessions offered and selected, age group of the Member, and ability as determined in the sole discretion of the Sports Academy (the “Membership”). The Membership types, options offered for any particular Membership, the amount of fees and other charges payable by Member, or Member’s Parent or Guardian as applicable, the suspension or termination of members, and all other matters affecting or relating to Member’s Membership shall be in the Sports Academy’s sole discretion.
Membership and Services. 1. A patient may “suspend”(1) his or her or family membership at their own discretion but must make DIME Medical aware of their intent to suspend membership in order to stop any automatic payments and as a general courtesy.
Membership and Services. In exchange for Patient’s payment of the Registration Fee (as defined in Section 2.a.) and the periodic Monthly Membership Fee (as defined in Section 2.b.), Patient is hereby enrolled as a member in the Practice’s direct primary care membership program (“DPCMP”), subject to the terms and conditions contained in this Membership Agreement. As a member of the DPCMP, Patient shall be eligible to receive certain primary care medical services (“Services”) provided at the Practice in accordance with the schedule in Appendix B attached hereto and incorporated herein by reference. The Practice will maintain a complete list of the Included Services/Excluded Services on its website, available at: xxxxxxxxxxxxxxxx.xxx. The Practice may add or discontinue a service to the list of Included Services in its sole discretion and will provide at least thirty (30) days’ advance written notice by emailing Patient and updating the Practice’s website of any change to the list of Included Services. If Patient does not have an email address, then Practice will text Patient at the mobile number provided by Patient in the registration materials and send Patient written notice in the mail to the last address provided by Patient to the Practice.
