Neither Practice nor its Physician(s) make any representations regarding third party insurance reimbursement of fees paid under this Agreement, and such reimbursement is not anticipated by this Agreement.

Non-Participation in Health Insurance. Neither practice, nor its physicians participate in any public or private health insurance plans, including Medicare. We do not make any representations regarding third party insurance reimbursement and such reimbursement is not anticipated by this Agreement. Pursuant to federal regulations, our physicians have elected “opt outstatus of Medicare participation. This means that Medicare cannot be billed for any Services performed under this Agreement. Further, you agree not to bill Medicare or attempt Medicare reimbursement for any such Services. If you are eligible for Medicare, or during the term of this Agreement you become eligible for Medicare, then Practice is required to obtain your understanding, memorialized by your signature, of our Private Medicare Contract. If you are (or become) Medicare eligible and choose not to sign our Private Medicare Contract, your membership will be automatically terminated and any remaining Fee will be prorated and refunded to you.
Non-Participation in Health Insurance. You acknowledge that neither the Practice, nor its Physicians participate in any public or private health insurance plans, including Medicare and Medicaid. Neither the Practice nor its Physicians make any representations regarding third party insurance reimbursement of Fees paid under this Agreement, and such reimbursement is not anticipated by this Agreement. If your health plan is a managed care product like an HMO or an EPO, including Medicare Advantage Plans and our local managed care plan, the plan generally will not cover ancillary services such as labs, imaging, and specialist referrals that the Practice orders. While we are happy to work with you, you understand that these plans often require re-evaluation and further workup through your assigned primary care physician in those plans.

Non-Participation in Health Insurance. For each year of this contract, it is agreed that the District will pay the amount of $4,500 for each employee who chooses not to participate in the health insurance program for the entire school year to be deposited into an individual annuity onbehalf of that employee. An employee is not eligible for the annuity in lieu of health insurance if he/she receives health insurance for any portion of a fiscal year. If an employee initially elects an annuity, but because of a qualifying life event requires insurance, the remaining annuity amount will be forfeited for the remainder of the fiscal year. Each employee who opts for the annuity in lieu of health insurance must contact the appropriate investment provider prior to December 1 of the current fiscal year to establish the account prior to funds being dispersed. A list of potential investors will be provided by District Office staff. If an account is not established prior to December 1 of the current fiscal year, the employee will forfeit the benefit amount of $4,500 for that fiscal year. It is further agreed that this annuity option shall be included in the amount credited toward retirement in accordance with existing regulations under both the TRS and the IMRF retirement systems. The Board will be held harmless concerning issues with TRS, IMRF and the IRS. The WCATS Co-Presidents will be notified via Board Meeting Summary when a new employee who is covered by the contract is hired or when a part-time employee becomes a full-time employee. This will be done within 15 school days of hiring or change.

  • Retiree Health Insurance Retired members of the Department receiving, or to receive City of Lincoln monthly pension checks, may participate in the group comprehensive health care plan for active City employees, provided that each retiree so desiring will execute the required forms in a timely fashion, and further provided that each retiree will be required to pay the full monthly cost at the current rates subject to any rate increases which may occur from time to time. Such payment will be made by payroll deduction from pension checks, or by direct payment in the case of an early retiree.

  • Ontario Health Insurance Plan The parties recognize that the method of funding OHIP has been changed from an individually paid premium to a system funded by an employer paid payroll tax. If the government, at any time in the future, reverts to an individually paid premium for health insurance, the parties agree that the Colleges will resume paying 100% of the billed premium for employees.

  • Health Insurance Plan (Excluding Summer Students Regardless of Wage Schedule Paid From) These employees shall be considered as a group in order that they may apply to participate in the Supplementary Plan and the Extended Health Benefit Plan at group rates. One hundred percent (l00%) of all premiums will be paid by the employees.   • State Employee Group Insurance Program (SEGIP) During the life of this Agreement, the Employer agrees to offer a Group Insurance Program that includes health, dental, life, and disability coverages equivalent to existing coverages, subject to the provisions of this Article. All insurance eligible employees will be provided with a Summary Plan Description (SPD) called “Your Employee Benefits”. Such SPD shall be provided no less than biennially and prior to the beginning of the insurance year. New insurance eligible employees shall receive a SPD within thirty (30) days of their date of eligibility.

  • Retirement Programs The Company agrees to provide Employees with the benefits under the Magna Group of Companies Retirement Savings Program as set out in the Employee Retirement Savings Program Booklets.
