Overview of AmeriCorps Promise Fellow Service. Each Promise Fellow follows the position description submitted with the Host Site’s application to host Promise Fellows. The position description provides an outline of service activities. DRAFT Each Host Site, with the support of the Promise Fellow, works towards the following goals: ● Create a Focus List (see chart on pg 1 for student numbers based on member slot type) of youth in grades 6-12 who demonstrate need for improvement based on attendance, student engagement, and/or work completion thresholds and report out data in the Alliance online reporting system. Continue to support Focus List students in the summer months. ● Coordinate programs and volunteers so that focus list youth in grades 6– 12 will demonstrate improvement in attendance, student engagement, and/or work completion. ● Set and locally validate attendance, student engagement, and work completion thresholds for youth participating in the program. Alliance suggested Early Warning Indicators can be found in Section 2, Part II. ● Provide targeted support to youth on the Focus List by intentionally engaging each student in Caring Adult, Service & Service Learning, and Out-of-School supports. ● Recruit and/or support community volunteers to support youth. The volunteers, including family members, work with youth participants in areas such as mentoring, tutoring, civic engagement, and college/career exploration. ● Meet regularly with the Youth Success Team (as defined in Section 1, Part III.F) to review data, identify youth to serve, track student progress, and determine which interventions to connect to individual students or groups of students.

  • Mastectomy Services Inpatient This plan provides coverage for a minimum of forty-eight (48) hours in a hospital following a mastectomy and a minimum of twenty-four (24) hours in a hospital following an axillary node dissection. Any decision to shorten these minimum coverages shall be made by the attending physician in consultation with and upon agreement with you. If you participate in an early discharge, defined as inpatient care following a mastectomy that is less than forty-eight (48) hours and inpatient care following an axillary node dissection that is less than twenty-four (24) hours, coverage shall include a minimum of one (1) home visit conducted by a physician or registered nurse.

  • Streets and Municipal Services 4.1 Off-Site Disturbance Any disturbance to existing off-site infrastructure resulting from the development, including but not limited to, streets, sidewalks, curbs and gutters, street trees, landscaped areas and utilities, shall be the responsibility of the Developer, and shall be reinstated, removed, replaced or relocated by the Developer as directed by the Development Officer, in consultation with the Development Engineer.
