Page Overview. Existing Child Placement Agreement Pages can be accessed via the associated Case Book and/ or Person Book, center drop down panel. Upon selecting Child Placement Agreement, the center panel will display all existing Child Placement Agreement pages that were created and/or last updated within the last 24 months. Each Child Placement Agreement page displays as: Child Placement Agreement - <Type> as the hyperlink; Last Name, First Name Middle Name as plain black text directly beneath the hyperlink as row 2 (NOTE: ; then Most Recent Effective Date and Terminate Date display in plain black text as row 3. Most Recent Effective Date and Terminate Date are field labels which will display at all times. Each hyperlink will launch the associated Child Placement Agreement Page in edit or view mode based on where the Case Book page was accessed from in combination with the users’ security. The 2 differences from Case Book to Person Book are 1) Person Book displays the associated Case ID for each Child Placement Agreement page in parenthesis, within the hyperlink, directly to the right of the Type; and 2) because Person Book is specific to one participant, if there is a current Child Placement Agreement (not terminated) it will display first and then all terminated Child Placement Agreement pages will display newest to oldest based on the very first Effective Date documented on the page. Another way of accessing existing Child Placement Agreement Page(s) is to navigate on the Desktop to the Case Folder icon and locate the Child Placement Agreement icon. All existing Child Placement Agreement pages will be displayed as hyperlinks that will launch the associated Child Placement Agreement page in edit or view mode based on where the outliner was accessed from in combination with the users’ security. This applies to the Desktop Outliner (Case and Participant View) and Search Outliner (Case tab – Case or Participant View, Person tab, Provider tab, and Worker tab). In accessing all existing Child Placement Agreement pages, they will display newest to oldest based on the very first Effective Date documented on the page. If more than one child has the same Effective Date, they will display based on Last Name, First Name, and Middle Name alphabetically. Page Overview. The Caregiver Supports tab allows users to identify and document supports for a caregiver. The tab requires that at least one record must be inserted and saved. When inserted, the Effective Date and narrative field are both required in order to save. In addition, the Emergency Contact narrative is required to save.

The purpose of this grant is to implement a Transformation Plan that has been developed through a local planning process and furthers the goals of the Choice Neighborhoods Program. The core goals of Choice Neighborhoods are:
