PATIENT AND FINANCIAL RECORDS. Manager shall maintain all files and records relating to the operation of Practice including, but not limited to, customary financial records and patient files. The management of all files and records shall comply with all applicable federal, state and local laws, statutes, rulings, orders, ordinances and regulations ("Laws"), and all files and records shall be located so that they are readily accessible for patient care, consistent with ordinary records management practices. Practice shall supervise the preparation of, and direct the contents of, patient medical records, all of which shall be and remain confidential and the property of Practice. Manager shall have reasonable access to such records and, subject to applicable Laws and accreditation policies, Manager shall be permitted to retain true and complete copies of such records. Manager hereby agrees to preserve the confidentiality of such patient medical records and to use the information in such records only for the limited purposes necessary to perform the Management Services and, within the limits of its responsibilities hereunder, to ensure that provision is made for appropriate care for patients of Practice.

Practice shall supervise the preparation of, and direct the contents of, patient medical records, all of which shall be and remain confidential and the property of Practice. Manager shall have reasonable access to such records and, subject to applicable Laws and accreditation policies, Manager shall be permitted to retain true and complete copies of such records at its expense. Manager hereby agrees to preserve the confidentiality of such patient medical records and to use the information in such records only for the limited purposes necessary to perform the Management Services and, within the limits of its responsibilities hereunder, to ensure that provision is made for appropriate care for patients of Practice.

PATIENT AND FINANCIAL RECORDS. AmeriPath shall maintain files and records relating to the operation of the Practice, including, but not limited to, customary financial records and patient files. Files and records shall be located to be readily accessible for patient care, consistent with customary records management practices. AmeriPath shall have reasonable access to files and records, and, subject to applicable laws, shall be permitted to retain copies of them. The Practice shall be responsible for the preparation of, and direct the contents of, patient medical records. All patient medical records shall remain the property of the Practice. The Practice shall be responsible for proper documentation of medical services provided by the Practice and the Practice Providers.

PATIENT AND FINANCIAL RECORDS. The following provisions shall apply to patient and financial records: Manager and Client acknowledge that Client may be a “covered entity” as that term is defined in the Health Insurance Portability and Accountability Act of 1996 (42 U.S.C. 1320d), as amended (“HIPAA”) and its accompanying regulations. Accordingly, the parties hereto agree to enter into a business associate agreement.
PATIENT AND FINANCIAL RECORDS. UDC shall maintain all files and records relating to the operation of the Practice, including, but not limited to, customary financial records and patient files. Notwithstanding the foregoing, the Practice shall supervise the preparation and direct the contents of patient dental records. All such patient dental records shall be and shall remain the property of Practice and the contents thereof shall be solely the responsibility of Practice. UDC shall have reasonable access to such records and, subject to applicable laws and regulations as necessary to perform his duties under this Agreement.

  • RECORDS AND FINANCIAL STATEMENTS The Advisor, in the conduct of its responsibilities to the Company, shall maintain adequate and separate books and records for the Company’s operations in accordance with GAAP, which shall be supported by sufficient documentation to ascertain that such books and records are properly and accurately recorded. Such books and records shall be the property of the Company and shall be available for inspection by the Board and by counsel, auditors and other authorized agents of the Company, at any time or from time to time during normal business hours. All financial statements that the Advisor delivers to the Company shall be prepared on an accrual basis in accordance with GAAP, except for special financial reports that by their nature require a deviation from GAAP. The Advisor shall liaise with the Company’s officers and independent auditors and shall provide such officers and auditors with the reports and other information that the Company so requests.

  • Medical Records Medical records relating to Trial Subjects that are not submitted to Sponsor may include some of the same information as is included in Trial Data; however, Sponsor makes no claim of ownership to those documents or the information they contain. c.

  • Books, Records and Financial Statements At all times during the continuance of the Company, the Company shall maintain, at its principal place of business, separate books of account for the Company that shall show a true and accurate record of all costs and expenses incurred, all charges made, all credits made and received and all U.S. income derived in connection with the operation of the Company’s business in accordance with generally accepted accounting principles consistently applied, and, to the extent inconsistent therewith, in accordance with this Agreement.
