The Fire Department Mechanic's, Fire Protection Technologist's, and pension will be provided on a "normal retirement age 65" basis. The Corporation shall pay the billed premiums for the following forms of insurance: Ontario Health Insurance Group Life Insurance Extended Health Care Hospital: semi-private Prescription Drugs: (excluding over the counter drugs) generic drugs only, unless the physician prescribes otherwise. Prepaid with Card and Long Term Disability Insurance maximum Dental : Shall be paid on the basis of the previous year’s Ontario Dental Association fee schedule. Dental coverage to provide for recall exams every nine months. Orthodontic: maximum per child per year lifetime maximum per adult. restorative: Reimbursement is limited to of the insured charges, to a maximum of per person per year.   • GROUP INSURANCE PLANS 15.01 The Company agrees to provide all full time employees with one (1) or more years of continuous service, a *Short Term Disability Benefit (S.T.D.) as generally described in Section B of a notice to all employees of the amended Benefit Plan dated May 1, 1993. *Payment for Short Term Disability shall begin on the third (3rd) consecutive day of absence. For those employees who have completed ninety (90) days of perfect attendance from the last date of return to work from an absence due to sickness or accident, shall be paid S.T.D. from the first (1st) day for the first covered absence following the ninety (90) days of perfect attendance.

  • I nsurance During the License Term, Licensee shall, at its own cost and expense, procure and continue in force such insurance policies as are required by Licensor. Such insurance shall, at a minimum include commercial general liability insurance with a combined policy limit of at least $1,000,000 or such other amount as is reasonably agreed to by the parties. Licensor shall be named as an additional named insured on all such policies of insurance. A renewal policy shall be procured not less than ten (10) days prior to the expiration of any policy. Each original policy or a certified copy thereof, or a satisfactory certificate of the insurer evidencing insurance carried with proof of payment of the premium, shall be deposited with Licensor prior to the commencement date of the term hereof and within ten (10) days of the each anniversary date thereafter. If possible and financially feasible, Licensee shall endeavor to have the foregoing insurance policy provide coverage for issues related to COVID-19, novel coronavirus, or similar issues. Licensee shall provide workers’ compensation and employer liability coverage as may be required by the State of Nebraska.

  • Long Term Care Insurance The University offers full-time faculty the opportunity to purchase Long-Term Care Insurance through a voluntary Long-Term Care Insurance policy. Faculty members are responsible for 100% of the premium, which may be remitted through payroll deduction.

  • Insurance Plans The Executive is eligible to participate in the life, health, dental, short and long-term disability plans made available to the employees of the Company pursuant to the terms and conditions of such plans.

  • Group Insurance 38.01 The Group Insurance Plan presently in effect shall remain in effect during the term of this Agreement.
