Preparation of RFP. Proposer shall submit the completed Request for Proposal (RFP) with appropriate attachments or explanatory materials. This is to include I) Letter of Transmittal: A one page letter which describes the main features and methods of the proposed system; 2) A complete proposal which shall include the following items submitted in two (2) separate lots: Lot 1, The Cost Proposal, shall contain: Purchasing and Contracting Form Representation and Certifications Pricing Schedule No premiums, rebates or gratuities permitted either with, prior to, or after any delivery of material is allowed. Any such violation will result in the cancellation and of material as applicable and or termination of contract. Quoted prices shall be firm. No rebates are to be part of any discounts in this RFP and continuation of contract.
Preparation of RFP. The respondent shall submit proposals in accordance with the public notice. Any proposal which contains any omissions, erasures, alterations, additions, irregularities of any kind, or items not called for which shall in any manner fail to conform to the conditions of public notice may be rejected. A proposal submitted by a partnership shall be executed in the partnership name and signed by a partner (whose title must appear under the signature). The official address of the partnership shall be shown below the signature. A proposal submitted by a limited liability company shall be executed in the name of the firm by a member and accompanied by evidence of authority to sign. The state of formation of the firm and the official address of the firm must be shown below the signature. A proposal submitted by an individual shall show the respondent’s name and official address. A proposal submitted by a joint venture shall be executed by each joint venture in the manner indicated on the bid form. The official address of the joint venture must be shown below the signature. It is preferred that all signatures be in blue ink with the names type or printed below the signature. Santa Xxxx County does not accept electronic signatures in proposal submissions. The proposal shall contain an acknowledgement of receipt of all Addenda, the numbers of which shall be filled in on the form. The address and telephone # for communications regarding the bid shall be shown. If the respondent is an out-of-state corporation, the bid shall contain evidence of respondent’s authority and qualification to do business as an out-of-state corporation in the State of Florida. A state contractor license # for the State of Florida shall also be included on the bid form. Respondent shall be licensed in accordance with the requirements of Chapter 489, Florida Statutes. The proposal shall be based upon the completion of the Work according to the drawings and specifications, together with all addenda thereto.
Preparation of RFP. Respondents shall submit the completed Request for Proposal (RFP) with appropriate attachments and a cover letter explaining your company’s history and qualifications to provide the Laundry Service to the County of Santa Xxxx. All attachments shall be identified with the Respondent’s name, RFP number and page number. No oral, telegraph, telephone, facsimile, electronic responses or photocopies will be accepted. RFPs must be completed in ink, typewritten, or word-processed.

Awarded vendor agrees that a violation of this condition shall be considered a material breach and may result in the cancellation of the purchase order at the TIPS Member’s discretion. Awarded vendor must identify any additional costs associated with compliance of this term. If no costs are specified, compliance with this term will be provided at no additional charge. Safety measures: Awarded vendor shall take all reasonable precautions for the safety of employees on the worksite, and shall erect and properly maintain all necessary safeguards for protection of workers and the public. Awarded vendor shall post warning signs against all hazards created by the operation and work in progress. Proper precautions shall be taken pursuant to state law and standard practices to protect workers, general public and existing structures from injury or damage.
