QAPP Implementation. This QAPP has been developed to provide a resource for facilities defined as dischargers and property owners for developing work plans that meet RWQCB data quality objectives (DQOs). To aide in the implementation of the QAPP procedures and to streamline the development of acceptable facility work plans, worksheets are included in Appendix A. The intent of these worksheets is to define the minimum information required to develop an acceptable quality plan and should be adapted as necessary to support specific project objectives. The following describes the individual worksheets:  Worksheet #1: Title Page, App roval Sheet, and Distribution List: If a stand -alone QAPP is developed, the QAPP must have a title and approval page with the relevant review and approval signatures. If the QAPP is included as a subsection of the work plan without a separate title page, t he title page must include the stamp of a California -registered geologist, or a California -registered civil engineer with at least 5 years of hydrogeologic experience.  Worksheet #2: Project Organization and Worksheet #3: Key Personnel, Responsibilities, Qu alifications, Contact Information: Quality planning must have as an output a description of the project organization in the form of an organization chart. The organization chart must show lines of authority and communication for the key stakeholders and pr oject personnel.
