Release of Child. Parent will provide a list of individuals authorized to pick up child from Center. Parent agrees to notify Center in advance, in writing, each day that anyone other than parent or one of Parent’s authorized representatives will pick up the child. Release of Child. I understand that my child will not be released to an authorized pick-up person or myself should the center staff suspect this person to be under the influence of drugs or alcohol. I further understand that my child must be in a proper seat restraint appropriate for his/her age and weight. I further understand that without a court order my child may be released to the parent or custodian whether listed on the application or not with the approval of the local authorities.
Release of Child. TFAB will not release a child to anyone whose name is not listed on the emergency form. TFAB should be notified in writing by the parent, guardian, or custodian if an emergency change must be made. Identification at pick-up will be required. Parents, guardians or legal custodians should agree to keep all emergency information up to date at the center.

If a child stays only a few hours a month, the charge is $8.00 per hour for (1) student, $10.00 per hour for (2) and $12.00 per hour for (3+). This will only cover before and after school care and not days when school is closed but care is available. Childcare will be available on some days when school is closed for K55th grade. Release of Child. Each parent or legal guardian will provide us with a list of individuals who are authorized to pick-up their child from camp (licensing form). Any changes to this form or to add additional authorized persons must be done in writing. You may drop off a note or email it to us from the email address you have on file to authorize your child to leave with a person who is not on your paperwork at the time of registration. This would include when your child goes on a play date, is picked up by a girl/boy scout leader, sports coach or anyone other than those you are listing upon registration. Such changes must state the specific authorized person’s name, day of pick-up or if the person is to be added to their general list of authorized representatives to pick-up your child from camp. Photo identification must be shown to our staff to verify that the proper person is picking up your child so please make the person picking up aware of this policy.

Supplies Parents are responsible for supplying • Formula/bottles • Diapers • Wipes • Sunscreen • A full change of clothing, including socks • Helmets, wrist protection, and knee and elbow pads for riding a scooter, bicycle, skateboard, skates, or rollerblades • Any other supplies your child may need All parents who have children who are not potty-‐trained will need to supply wipes and diapers. Please be aware when your child’s supplies are running out. If it becomes necessary for us to supply wipes or diapers for your child, reimbursement will be expected no later then your next payment date. Transitioning from diaper to underwear will be based on the needs of the child and a plan made by the parent and provider. Please have your child clean and dressed for play. We expect your child to arrive dressed for the day and dressed appropriately according to weather conditions, including snow clothing. We will be going outside daily weather permitting. All personal belongings should be marked with your child’s name. The providers are not responsible for your child’s clothing or personal items brought into our home. Sunscreen must be supplied and in cases where no sunscreen is available the provider will use the sunscreen, with a SPF of 30 that Xxxxxx’s House has on hand. Please bring a hat for your infants and they will also be provided with shade so they will be protected from sun exposure.
