Requesting Leave. To request leave from the sick leave bank the employee must submit a written request to the Superintendent detailing the nature of the illness, the number of sick bank days being requested, and include medical certification supporting the need for leave. The Superintendent shall develop a form to use for this purpose.

The Superintendent shall develop leave on a form approved by the Superintendent and Association. It is understood that the information provided on the written request, information found in the submitted medical certification, and prior attendance history shall be shared with the sick bank committee member in order to use consider the request for this purposeleave from the sick leave bank.
