Shareholder's Submissions. Shareholder herewith delivers to the Corporation:
a. detailed plans, specifications and drawings of the Work, including a room- by-room list of all alterations to be undertaken, and detailed plans and specifications (the "Plans") prepared by a licensed architect or engineer (if the nature of the alteration so requires), which shall not be modified by the Shareholder after they are approved by the Corporation's architect or engineer or Superintendent (the "Corporation's Designated Representative") without the Corporation's Designated Representative's subsequent approval.
b. a check with respect to the security payable in connection with this Agreement in the sum of $5,000.00 or ten (10%) percent of the cost of the Work, whichever is greater but in no instance shall exceed $15,000, payable to the Corporation in accordance with paragraph 13 of this Agreement, if applicable.
c. a check for $350.00 payable to XXXXXXX-XXXXX, INC., managing agent for the Building (the "Managing Agent") for coordination and review of the instant agreement, plans, collection of security and to report to the Corporation as necessary.
d. a check payable to XXXXXXX XXXXX DBA XXXXX ASSOCIATES, the Corporation’s engineer in the amount of $500.00 as a retainer against said engineer’s services which will be billed at the rate of $185/hour.
Shareholder's Submissions. Shareholder herewith delivers to the Corporation:
a. detailed plans, specifications and drawings of the Work, including a room-by- room list of all alterations to be undertaken, and if required by the Corporation, detailed plans and specifications (the "Plans") prepared by a licensed architect or engineer (if the nature of the alteration so requires), which shall not be modified by the Shareholder after they are approved by the Corporation's architect or engineer (the "Corporation's Designated Engineer") without the Corporation's Designated Engineer's subsequent approval.
b. a check with respect to the security payable in connection with this Agreement in the sum of $5,000.00 payable to the Corporation in accordance with paragraph 13 of this Agreement, if applicable.
c. a check in the sum of $300.00 payable to Kyrous Realty Group, managing agent for the Building (the "Managing Agent") as a processing fee in connection with this request and the Work, if applicable.
Shareholder's Submissions. Together with this Agreement, Shareholder is delivering to the Corporation:
a. detailed plans, drawings and specifications for the equipment proposed to be installed and/or the alterations proposed to be made which, if so required by the Corporation, have been prepared by a licensed architect or engineer. Such plans, drawings and specifications include a room-by-room list of the equipment to be installed and the alterations to be made. A detailed list of all such plans, drawings and specifications are annexed hereto as Exhibit "A."
b. a check in the sum of $__________ payable to the Corporation for the security deposit required to be posted by the Shareholder as provided for in Paragraph 14 of this Agreement (the "Security Deposit"), if required by the Corporation;
c. a check in the sum of $_______ payable to ________________(Managing Agent), the managing agent for the Building (the "Managing Agent"), as a processing fee in connection with this Agreement, if required by the Corporation; and
d. a check in the sum of $________________ payable to the order of the Corporation, as payment "on account" of the fees, disbursements, charges and costs (including but not limited to attorney and engineering fees) to be incurred by the Corporation in connection with the preparation of this Agreement, review of the plans, inspection and monitor of the Work (hereinafter defined), enforcement of this Agreement, and such other expenses incurred by the Corporation (the "Review Deposit").
Shareholder's Submissions. Together with this Agreement, Shareholder is delivering to the Corporation:
a. Detailed plans, drawings and specifications for the equipment proposed to be installed and/or the alterations proposed to be made which, if so required by the Corporation, have been prepared by a licensed architect or engineer. Such plans, drawings and specifications include a room-by-room list of the equipment to be installed and the alterations to be made. A detailed list of all such plans, drawings and specifications are annexed hereto as Exhibit “A.”
Shareholder's Submissions. Together with this Agreement, Shareholder is delivering to the Corporation:
a. detailed plans, drawings and specifications for the equipment proposed to be installed and/or the alterations proposed to be made which, if so required by the Corporation, have been prepared by a licensed architect or engineer. Such plans, drawings and specifications include a room-by-room list of the equipment to be installed and the alterations to be made. A detailed list of all such plans, drawings and specifications are annexed hereto as Exhibit "A."
b. a check in the sum of $300 (for non-structural renovations) or $700 (for structural renovations) payable to Garthchester Realty (Managing Agent), the managing agent for the Building (the "Managing Agent"), as a processing fee in connection with this Agreement, if required by the Corporation; and
c. a refundable deposit of $3000 (for non-structural renovations) or $5000 (for structural renovations) payable to 3777 Independence Corp. (the Building), to be returned upon completion of the alteration without damages occurring to common elements or costs to the Building.
Shareholder's Submissions. Shareholder herewith delivers to the Corporation:
a. detailed plans, specifications and drawings of the Work, including a room-by-room list of all alterations to be undertaken, and if required by the Corporation, detailed plans and specifications (the "Plans") prepared by a licensed architect or engineer, which shall not be modified by the Shareholder after they are approved by the Corporation's architect or engineer without the architect's or engineer's subsequent approval.
b. a check with respect to the security payable in connection with this Agreement in the sum of $2,000.00 (Two thousand dollars) payable to 308 Owners Corp. in accordance with paragraph 16 of this Agreement, if applicable.
c. a check in the sum of $250.00 (Two hundred and fifty dollars) payable to Xxxxxxx Xxxxxxx Property Management, managing agent for the Building (the "Managing Agent") as a processing fee in connection with this request and the Work, if applicable. *COSMETIC WORK - Note to Shareholder: This form of Alteration Agreement is to be used for structural alterations to the Apartment (i.e., removing all or part of a wall, changing the plumbing or electrical lines, or replacing windows). If you are just doing cosmetic work (i.e., painting, wallpapering, changing bathroom, kitchen or lighting fixtures without altering the plumbing or electrical lines) change the deposit requirement in 1(b) to $1,000.00 and delete paragraphs 1(a), 2, 3, 8, 12, 14, 23, 24, 26 and 29.

a. detailed plans, specifications and drawings of the Work, including a room- by-room list of all alterations to be undertaken, and detailed plans and specifications (the "Plans") prepared by a licensed architect or engineer (if the nature of the alteration so requires), which shall not be modified by the Shareholder after they are approved by the Corporation's architect or engineer or Superintendent (the "Corporation's Designated Representative") without the Corporation's Designated Representative's subsequent approval.
b. a check with respect to the security payable in connection with this Agreement in the sum of $5,000.00 or ten (10%) percent of the cost of the Work, whichever is greater but in no instance shall exceed $15,000, payable to the Corporation in accordance with paragraph 13 of this Agreement, if applicable.
c. a check for $350.00 payable to XXXXXXX-XXXXX, INC., managing agent for the Building (the "Managing Agent") for coordination and review of the instant agreement, plans, collection of security and to report to the Corporation as necessary.
Shareholder's Submissions. Shareholder herewith delivers to the Corporation:
a. detailed plans, specifications and drawings of the Work, including a room-by- room list of all alterations to be undertaken, and if required by the Corporation, detailed plans and specifications (the "Plans") prepared by a licensed architect or engineer (if the nature of the alteration so requires), which shall not be modified by the Shareholder after they are approved by the Corporation's architect or engineer (the "Corporation's Designated Engineer") without the Corporation's Designated Engineer's subsequent approval.
b. a letter to the Corporation from a licensed engineer or architect, which letter shall certify that the electrical loads required as a result of the Work, if any, will not be in excess of the present electrical capacity of Shareholder's apartment(s). Said letter shall also certify that the Work will not adversely affect the building's electrical service. (See Exhibit A, Electrical Upgrade Policy.)
c. a check with respect to the security deposit payable in connection with this Agreement in the sum of $1500 payable to the Corporation in accordance with paragraph 13 of this Agreement, if applicable.
d. a check payable to the Managing Agent for the building in the sum of $150 as a processing fee in connection with this request and the Work.
e. a check payable to the Corporation in the sum of $150 as a consideration for Corporation and building services in connection with the Work.
Shareholder's Submissions. Shareholder herewith delivers to the Corporation:
a. detailed plans, specification and drawings of the Work that conform to the Corporation’s Design Criteria, including a room‐by‐room list of all alterations to be undertaken, and if required by the Corporation, detailed plans and specifications (the “Plans”) prepared by a licensed architect or engineer (if the nature of the alteration so requires), which shall not be modified by the Shareholder after they are approved by the “Corporation’s Designated Engineer”) without the Corporation’s Designated Engineer’s subsequent approval.
b. a check with respect to the security payable in connection with this Agreement in the sum of $ payable to the Corporation in accordance with paragraph 13 of this Agreement, if applicable. (Note: The amount of the deposit is to be determined by the managing agent after the plans are approved. The deposit will not be less than $5,000.00).
c. a check in the sum of $500 payable to Xxxx Realty Management Corp., managing agent for the Building (the “Managing Agent”) as a processing fee in connection with this request.
d. a check in the amount of $50 payable to the Xxxxxx Apartment Corp. as the administrative fee to the Corporation.
Shareholder's Submissions. Shareholder herewith delivers to the Board of Directors:
A. detailed plans, specifications and drawings of the Work, including a room-by-room list of all alterations to be undertaken, and if required by the Board of Directors, detailed plans and specifications (the “Plans”) prepared by a licensed architect or engineer (if the nature of the alteration so requires), which shall not be modified by the Shareholder after they are approved by the Cooperative Corporation’s architect or engineer (the “Cooperative Corporation’s Designated Engineer”) without the Cooperative Corporation’s Designated Engineer’s subsequent approval.
B. a check with respect to the security payable in connection with this Agreement in the sum of $1,000 payable to the Cooperative Corporation in accordance with paragraph 13 of this Agreement, if applicable.
C. a check in the sum of $250.00 payable to Pride Property Management Corp., managing agent for the Building (the “Managing Agent”) as a processing fee in connection with this request and the Work, if applicable.
