Sick Leave and Short Term Disability Leave. ‌‌ A short-term disability leave may be granted to employees who are required to be absent from work due to sickness or injury unrelated to work. In order to qualify for short-term disability leave, employees are required to notify their immediate supervisor of their absence from work due to sickness or disability. An employee must be absent from work for more than five (5) consecutive working days prior to being eligible for short term disability. Employees can utilize vacation, personal emergency days, or unpaid time during the five (5) consecutive sick days to bridge the waiting period. Applications for short term disability are adjudicated by a third party, and therefore the Town is not privy to the employee’s confidential medical information. Once the employee is approved for short-term disability the Corporation will reimburse the employee for the use of any of the bridge options above. Employees will be eligible for a maximum of twenty-six (26) weeks’ of wages per single disabling event or related disabling events, on the condition that the employee continues to provide the employer and/or adjudicator with updated medical documentation as requested. The Town will start the LTD process prior to the elimination process, as prescribed by the benefit carrier, to ensure a continuation of pay. Should the claim be denied, the employee is expected to return to work immediately.

  • Short-Term Disability Leave In order to access short-term disability leave, medical confirmation may be requested and shall be provided on the form attached as Appendix “C” to this Agreement. In either instance where an Employee does not provide medical confirmation as requested, or otherwise declines to participate and/or cooperate in the administration of the Sick Leave Plan, access to compensation may be suspended or denied. Before access to compensation is denied, discussion will occur between the union and the school board. Compensation will not be denied for the sole reason that the medical practitioner refuses to provide the required medical information. A school board may require an independent medical examination to be completed by a medical practitioner qualified in respect of the illness or injury of the Board’s choice at the Board’s expense. In cases where the Employee’s failure to cooperate is the result of a medical condition, the Board shall consider those extenuating circumstances in arriving at a decision.

  • Short Term Disability The Employer agrees to provide Short Term Disability benefits to all active full-time employees from the first (1st) day of an accident or the first (1st) full-time day of hospitalized or the fourth (4th) day of sickness. The Plan will pay sixty-six and two thirds percent (66 2/3%) of basic earnings for the first two (2) weeks, then Unemployment Insurance will pay fifteen (15) weeks, then the Plan will resume payments for thirty-five (35 weeks).

Short term disability benefits for disabilities resulting from non-occupational illness or injury, shall be paid at the rate of 70% of the teacher’s regular daily rate, subject to all applicable deductions. A teacher may choose to save up to five (5) accumulated temporary leave days. Following the exhaustion of temporary leave, there is a five day waiting period before short term disability benefits begin. The five day waiting period will be waived for absences greater than 30 calendar days and short term disability payments shall be paid retroactively.

  • Maternity Disability Leave 14.1.13.1 This leave commences with the onset of disablement due to pregnancy. The employee may claim sick leave pay and/or extended disability pay for no more than that limited period of time when the employee’s physician certified in writing on the form provided by the District that she was actually physically disabled from performing her duties because of pregnancy, miscarriage, childbirth, or recovery there from.

  • Short Term Sick Leave 8. Each school year, an employee absent beyond the eleven (11) sick leave days paid at 100% of salary, as noted in section 2 above, shall be entitled up to an additional one hundred and twenty (120) days short term sick leave to be paid at a rate of 90 per cent of the employee’s regular salary if the employee is absent due to personal illness including medical appointments as per the collective agreement provisions and practices in effect as at August 31, 2012.

  • Extended Disability Leave a. Due to a medical disability, an employee shall be granted an extended leave of absence without pay (subject to Paragraph d hereof) if any one of the following conditions exists:
