Common use of Sick Leave Form Clause in Contracts

Sick Leave Form. (a) An employee absent from work through illness or injury will within three days of returning to work from the initial absence, submit a fully completed Leave Request Form. (b) The Employer may also request a fully completed report from a qualified medical practitioner in one or more of the following circumstances: (1) where it appears that a pattern of consistent or frequent absence from work is developing; (2) where the employee has been absent for five consecutive scheduled days of work; (c) Benefits will cease to be paid when an employee fails to provide satisfactory evidence of medical disability during the benefit period. (d) Where the Employer requests a report from a medical practitioner the Employer will bear the costs incurred in obtaining same.
