It is your responsibility to take keep medication safe. You are not to share medication with anyone or adjust doses without provider approval. If you decide to stop medication, you should contact your doctor. • If you or anyone in your family has ever had drug or alcohol problems, you have a higher chance of getting addicted to this medicine. • Your doctor can only prescribe this medicine if you do not use illegal drugs. • If you do not use this medication exactly as prescribed, you risk hurting yourself and others. • You cannot increase your medicine dose without being told to do so by your doctor. • This medicine will not be refilled early. • You will take full responsibility for your medicine. If you have a child or pet in the house or live with someone who has a substance use disorder, you will keep your medications in a secure location. • When you travel, keep the medication in the original prescription bottle with the label that has the name of the medication, dose and name of the doctor who wrote that prescription. o Do not put the controlled substance in a pill dispenser when traveling. • The medicine will not be replaced if it is stolen or lost. • You will not share or give this medicine to other people. • Provide a urine or blood sample when asked to help monitor your treatment. Our clinic policy requires regular testing. • Bring your pill bottles with all pills to the pharmacy for a pill count and verification if asked. • Go to appointments and tests set up by your doctor. Metabolic or drug induced: - Haloperidol 500micrograms to 5mg oral nocte, (or subcutaneous 5 - 10 mg/24 hrs), or:Levomepromazine, oral or sc, from 6.25 mg od. Intestinal obstruction - contact Specialist Palliative Care Team. Treat underlying cause: e.g. drugs/ hypercalcaemia/ infection/ dehydration/ pain/ urinary retention, etc. Midazolam - from 2.5 mg stat, every 2 - 4 hours, or:Haloperidol - 2.5 to 10 mg, oral or subcutaneous. Midazolam – from 10mg to 30 mg subcutaneous over 24hrs via syringe driver If still restless, seek specialist palliative care advice. Consider when symptoms include dehydration, constipation, confusion, nausea & vomiting and increased thirst. Treat symptomatic patients with an adjusted serum calcium > 2.6 mmol/L with intravenous fluids and iv bisphosphonate – contact Specialist Palliative Care Team. Hyoscine hydrobromide - 600 mcg subcutaneously stat, 600 micrograms – 2.4mg over 24 hours sc via syringe driver. Stimulants. Physician will use stimulants only for patients with ADHD. Adult Attention-Deficit/Hyperactivity Disorder (ADHD) is a mental health disorder that needs stimulants for maintenance care.  Physician will use ADHD medication only for patients who have psychological testing done to support the diagnosis or documented treatment history of childhood ADHD.  Physician needs medical records to support previous care on ADHD medication.  Physician will offer safer medication alternatives such as Wellbutrin, Straterra and Intuniv.  Physician will not choose more than one medication for treating ADHD. Patients with any other active substance abuse will not be treated with stimulants simultaneously.  Physician will not prescribe beyond the listed medication and its dosage or will only use FDA approved medication for ADHD within the recommended dosage. Prescription for controlled substances, requirements: ALL prescriptions will include  Dated and signed the day it is issued.  Full name and address of the prescriber.  Full name and address of the patient.  DEA number of the prescriber (who is authorized to prescribe the said drug).  Written signature of the prescriber.  Written in ink or indelible pencil or typewritten.  Specify drug name, strength, dosage and form.  Specify quantity.  Include direction for use.  Valid medical reason.  If electronic, will comply with the e-prescribing guidelines from the federal government.  No more than 2 refills. All patients will be seen within 90 days. All Schedule II stimulant medication prescriptions are also limited:  No refills.  No more than a 30-day supply  Only one prescription per prescription blank.  Schedule II prescriptions expire in 90 days.  Include both a written and numerical notation of quantity.

The Board considered the estimated costs to XXXX, and pre-tax profits realized by XXXX, from advising the DWS Funds, as well as estimates of the pre-tax profits attributable to managing the Fund in particular. The Board also received information regarding the estimated enterprise-wide profitability of DIMA and its affiliates with respect to all fund services in totality and by fund. The Board and the Fee Consultant reviewed XXXX’s methodology in allocating its costs to the management of the Fund. Based on the information provided, the Board concluded that the pre-tax profits realized by XXXX in connection with the management of the Fund were not unreasonable. The Board also reviewed certain publicly available information regarding the profitability of certain similar investment management firms. The Board noted that, while information regarding the profitability of such firms is limited (and in some cases is not necessarily prepared on a comparable basis), DIMA and its affiliates’ overall profitability with respect to the DWS Funds (after taking into account distribution and other services provided to the funds by XXXX and its affiliates) was lower than the overall profitability levels of most comparable firms for which such data was available. Economies of Scale. The Board considered whether there are economies of scale with respect to the management of the Fund and whether the Fund benefits from any economies of scale. The Board noted that the Fund’s investment management fee schedule includes fee breakpoints. The Board concluded that the Fund’s fee schedule represents an appropriate sharing between the Fund and DIMA of such economies of scale as may exist in the management of the Fund at current asset levels.

If your Account is in default, we may close your Account or terminate or suspend your credit privileges without notice and require full payment of your outstanding balance immediately. We can also begin collection activities. To the extent permitted by law, if you are in default because you have failed to pay us, we will require you to pay our collection costs, attorneys’ fees, court costs and all other expenses of enforcing our rights under this Agreement. Arbitration of Disputes This Agreement contains an Arbitration Provision immediately below. Within 90 days after the date we open your Account, you may elect to reject the Arbitration Provision by providing a written notice of your intention to do so to the address: Legal Division (745), Banco Popular de Puerto Rico, P.O. Box 362708, San Xxxx, Puerto Rico 00936-2708. Unless you submit your rejection or to the extent permitted by applicable law, the Arbitration Provision will have a substantial impact on how any legal claims we have against each other are resolved if either you or we elect arbitration. For example, disputes subject to arbitration are decided by a neutral arbitrator and not a judge or jury. This Arbitration Provision will not apply to, or be enforceable against, covered borrowers under the protection of the Military Lending Act. This Arbitration Provision shall not preclude you from pursuing or participating in a class action in court. IMPORTANT WAIVERS: IF YOU OR WE ELECT TO ARBITRATE A CLAIM, YOU AND WE BOTH WAIVE THE RIGHT TO: (1) HAVE A COURT OR A JURY DECIDE THE CLAIM; OR (2) OBTAIN INFORMATION EXCEPT AS PROVIDED HEREIN.   • Students Payments which a student or business apprentice who is or was immediately before visiting a Contracting State a resident of the other Contracting State and who is present in the first-mentioned State solely for the purpose of his education or training receives for the purpose of his maintenance, education or training shall not be taxed in that State, provided that such payments arise from sources outside that State.
