Submission of Additional Information. If the employee disagrees with the determination regarding the employee’s request for disability leave benefits, the employee may submit additional information to the Employer. Such information must be submitted within 30 days of the date of the notification of the determination or within 30 days of the ending date of approved disability benefits, whichever is later. The Employer shall, within five (5) days of receipt of such additional information, forward it to the Director of Administrative Services. By exercising such right to submit additional information, the employee does not waive his or her right to file an appeal pursuant to Section 11. The appeal must then be filed within 30 days of notification of the decision based on the submission of additional information.

The grievance must then be filed within thirty (30) days of notification of the decision based on the submission of additional information. If the employee chooses not to submit additional information, then he/she may file a grievance. Except as modified herein, any grievance arising under this section will be filed in accordance with the procedures contained in Article 20 of this Agreement, and shall be initiated at Step 3 of the Grievance Procedure.

Such information must be submitted within twenty (20) days of the date of the notification of the determination or within twenty (20) days from the ending date of approved disability benefits, whichever is later. The Appointing Authority shall, within five (5) days of receipt of such additional information, forward it to the Director of DAS or designee. Where extenuating circumstances prevent an employee from providing additional information within the required time frame, a written statement explaining such extenuating circumstances must be filed within a reasonable time after the twenty (20) day time period has expired. Appropriate extenuating circumstances shall be accepted as an extension of the time limit to provide additional information.
Submission of Additional Information. In addition to any other reports required of Retailer by the terms of this Agreement, Retailer shall submit to Kincaid sucx xxxxx reports and financial information as Kincaid frox xxxx to time may require. Specifically, Kincaid shaxx xx xrovided upon its request with personal financial statements of any person or persons having, or wishing to acquire, a material financial interest in Retailer's business.
Submission of Additional Information. The Claims Reviewer may require ------------------------------------ any Claimant to furnish such information as may reasonably be needed by the Claims Reviewer to process the Claimant's application and reach a decision upon such application.
