To Discontinue All Insurance Coverage. Insurance coverage as listed above shall automatically terminate the following month after the approved leave begins (the exact date to be determined by the carrier) unless the employee chooses option 2. An employee’s cost of insurance while on non-FMLA leave of thirty (30) days or longer begins thirty (30) days after an employee has exhausted his/her Paid Days Off (PDO), personal and compensatory leave banks. The Employer will begin counting the thirty (30) days on the first day that an employee receives no paid time or when the waiting period for disability benefits has been completed, whichever is earlier. Any Illness Leave Bank hours do not alter in any manner the cost of benefits while on a leave of thirty (30) days or more.

  • General Provisions for all Insurance Coverage 8.24.1 Without limiting Contractor's indemnification of County, and in the performance of this Contract and until all of its obligations pursuant to this Contract have been met, Contractor shall provide and maintain at its own expense insurance coverage satisfying the requirements specified in Paragraphs 8.24 and 8.25 of this Contract. These minimum insurance coverage terms, types and limits (the “Required Insurance”) also are in addition to and separate from any other contractual obligation imposed upon Contractor pursuant to this Contract.   • INDUSTRIAL INSURANCE COVERAGE The Contractor shall comply with the provisions of Title 51 RCW, Industrial Insurance. If the Contractor fails to provide industrial insurance coverage or fails to pay premiums or penalties on behalf of its employees, as may be required by law, Agency may collect from the Contractor the full amount payable to the Industrial Insurance accident fund. The Agency may deduct the amount owed by the Contractor to the accident fund from the amount payable to the Contractor by the Agency under this contract, and transmit the deducted amount to the Department of Labor and Industries, (L&I) Division of Insurance Services. This provision does not waive any of L&I’s rights to collect from the Contractor.
