TRANSITIONAL HOUSING SERVICES. CONTRACTOR shall provide a six (6seven (7) 31 1. An alcohol and drug-free, supervised, twenty-four (24) hour living environment for 32 Participants who are currently participating in treatment at a COUNTY-contracted Narcotic 34 Disorder Outpatient Clinics or Mental Health program and who have no available housing. Although 35 CONTRACTOR provides no direct counseling or treatment services to Participant, CONTRACTOR 36 shall encourage and provide structured daily activities; such as health and fitness recreation, providing 37 Participant responsibility for daily household duties, including food preparation, house cleaning, and 1 basic household operations. In addition, CONTRACTOR shall encourage Participant to live productive, 4 volunteering in the community. 5 2. House Rules for standards of conduct for all Participants shall be established which shall 6 include mandatory participation in a COUNTY-operated or COUNTY-contracted outpatient treatment 7 program requiring at least three (3) sessions per month. Proof of participation shall be documented in 9 treatment program or arrange discharge of any Participant who was discharged from outpatient treatment 10 as appropriate. CONTRACTOR has authority to discharge any Participant who violates house rules. 11 Said House Rules shall be enforced by CONTRACTOR’s House Manager, other paid staff, intern(s) or 12 volunteer(s), as designated by CONTRACTOR’s Executive Director. 13 3. Provision of three (3) meals per day. Such meals shall be nutritious and appropriate to the 14 health needs of the Participant. 15 4. Provision of laundry facilities at no cost to the Participant. 16 5. Provision of toiletry articles appropriate to the health and grooming needs of the Participant. 17 6. Provision of information regarding public transportation, which shall include bus schedules, 18 so that Participant may participate in substance use disorder treatment programs. . CONTRACTOR shall 20 7. PARTICIPANT SUPERVISION 21 a. CONTRACTOR shall provide on siteonsite supervision of all Participant activities 25 b. A house log shall be maintained on each shift and supervised by a designated program 26 staff person. TRANSITIONAL HOUSING SERVICES. Transitional Housing Units (THU) are provided by Asian Americans for Community Involvement (AACI) for Asian CalWORKs women and their children who are homeless or in need of a safe place to live while attending CalWORKs outpatient behavioral health services. No treatment is provided at the THU, clients are required to be in CalWORKs outpatient treatment while they are staying at the THU.

  • Transitional Services Upon cancellation, termination, or expiration of the Contract for any reason, the Contractor shall provide reasonable cooperation, assistance and Services, and shall assist the Department to facilitate the orderly transition of the work under the Contract to the Department and/or to an alternative contractor selected for the transition upon written notice to the Contractor at least thirty (30) business days prior to termination or cancellation, and subject to the terms and conditions set forth in the Contract.

  • Educational Services Any service or supply for education, training or retraining services or testing including: special education, remedial education; cognitive remediation; wilderness/outdoor treatment, therapy or adventure programs (whether or not the program is part of a Residential Treatment facility or otherwise licensed institution); job training or job hardening programs; educational services and schooling or any such related or similar program including therapeutic programs within a school setting.

  • Optional Services To the extent that the Fund elects to engage the Transfer Agent to provide the services listed below the Fund shall engage the Transfer Agent to provide such services upon terms and fees to be agreed upon by the parties: (a) Corporate actions (including inter alia, odd lot buy backs, exchanges, mergers, redemptions, subscriptions, capital reorganization, coordination of post-merger services and special meetings).

Melbourne Archdiocese Catholic Schools Ltd (MACS) governs the operation of MACS schools and owns, governs and operates the School. 1.2 Parents and guardians, as the first educators of their children, enter into a partnership with the Catholic school to promote and support their child’s education. Parents and guardians must assume a responsibility for maintaining this partnership by supporting the school in the provision of education to their children within the scope of School's registration and furthering the spiritual and academic life of their children.

  • Supportive Services 2.1. Case Management Access Shelter Providers are required to have case management available to participants on site. Participation within case management is voluntary to program participants, however all participants must be offered case management and must be engaged on an ongoing basis to encourage participation. Shelter Providers should recognize that it may take multiple contacts before a participant is ready to engage. Shelter Providers must ensure case management services are participant-centered to individual needs. Programs must provide space for the provision of case management that works to create as much privacy and confidentiality as possible.

  • Hospice Services Services are available for a Member whose Attending Physician has determined the Member's illness will result in a remaining life span of six months or less.

  • Transitional Services Agreement Seller shall have executed and delivered the Transitional Services Agreement.

  • Training Services Training Services may include pre-packaged training Products, and/or the development or customization of training programs as requested, including Live Training, Computer Based/Multi-Media Training which encompasses Internet-Delivered Training, and/or Video Based Training.

  • COVERED HEALTHCARE SERVICES This section describes covered healthcare services. This plan covers services only if they meet all of the following requirements: • Listed as a covered healthcare service in this section. The fact that a provider has prescribed or recommended a service, or that it is the only available treatment for an illness or injury does not mean it is a covered healthcare service under this plan. • Medically necessary, consistent with our medical policies and related guidelines at the time the services are provided. • Not listed in Exclusions Section. • Received while a member is enrolled in the plan. • Consistent with applicable state or federal law. We review medical necessity in accordance with our medical policies and related guidelines. Our medical policies can be found on our website. Our medical policies are written to help administer benefits for the purpose of claims payment. They are made available to you for informational purposes and are subject to change. Medical policies are not meant to be used as a guide for your medical treatment. Your medical treatment remains a decision made by you with your physician. If you have questions about our medical policies, please call Customer Service. When a new service or drug becomes available, when possible, we will review it within six (6) months of one of the events described below to determine whether the new service or drug will be covered: • the assignment of an American Medical Association (AMA) Current Procedural Terminology (CPT) code in the annual CPT publication; • final Food and Drug Administration (FDA) approval; • the assignment of processing codes other than CPT codes or approval by governing or regulatory bodies other than the FDA; • submission to us of a claim meeting the criteria above; and • generally, the first date an FDA approved prescription drug is available in pharmacies (for prescription drug coverage only). During the review period, new services and drugs are not covered. For all covered healthcare services, please see the Summary of Medical Benefits and the Summary of Pharmacy Benefits to determine the amount that you pay and any benefit limits.
