HEALTH AND FITNESS. 8.2.1 To ensure that an Employee is medically fit to carry out duties in a satisfactory and safe manner the Employee will, if required, undergo a medical examination or health assessment as arranged with the Employer’s Occupational Physician for purposes of National Health Assessment for the Rail Industry.
8.2.2 The Employer will pay the costs of any medical examination or assessment conducted by the Employer’s Occupational Physician. However, subject to any policy to the contrary, the Employee is responsible for any costs associated with any treatment of a condition identified by the Employer’s Occupational Physician.
8.2.3 The Employee will, as required, undergo drug and alcohol testing in accordance with the Employer’s policies on the safety of personnel working on or about the railway system.
8.2.4 The Employee will not be required to undergo a medical examination for the purposes of the National Health Assessment for the Rail Industry while such Employee is on workers compensation, except and only when an Employee returns from workers compensation and is medically cleared to recommence paid remuneration or a circumstance where a health assessment is required for the purpose of alternative duties.
HEALTH AND FITNESS. Good health and fitness of its employees is in the best interest of the University. Additionally, good health and fitness reduces absenteeism and prevents over utilization of employee medical plans. Therefore, the University will provide bargaining unit members the opportunity to utilize fitness facilities at a reduced cost to the employee. The University will also offer, from time to time, special rates and promotions for bargaining unit employees. Employees may use fitness facilities at times other than their regular working hours and when off duty. Users will be required to observe the rules and protocols required of any other users of the facilities.
HEALTH AND FITNESS. Effective July 1, 1991, employees shall be eligible to use the South Portland Municipal Pool facility and Municipal Golf Course during the regular operating hours of each respective facility at no charge to the employee for membership or daily fees. The fee shall be waived only for the employee, not the employee's spouse or family. Hours spent at either facility shall not be considered hours worked.

I represent and warrant that I am physically capable of participating in the Activities and do not have any medical condition, injury, or impairment that may affect my ability to safely participate. I acknowledge that it is my responsibility to consult with a physician prior to participating in the Activities if I have any concerns about my health or fitness.

I acknowledge that it is my responsibility to consult with a physician prior to the Minor's participation in the Activities if I have any concerns about their health or fitness.
HEALTH AND FITNESS. Your continuing employment is conditional upon you maintaining the necessary level of health and fitness to carry out the full range of your duties safely. To this end, you are required to cooperate with us in relation to managing any health related concerns which may affect your ability to perform your role safely.
HEALTH AND FITNESS. 28 In order to promote the fitness and health of employees of this unit, the City agrees to waive the 29 membership fees and daily user fees for the South Portland Municipal Golf Course, and 30 Municipal Pool. Employees shall be permitted to utilize such facilities during the normal 31 operating hours of those municipal fitness facilities.
HEALTH AND FITNESS.
39.1 A firefighter joining the Brunswick Fire Department after July 1, 1984 shall be tested for and maintain minimum physical fitness requirements.
39.1.1 A Physical Fitness Test (PFT) will be given no later than November 1 of each contract year.
39.1.2 The PFT will be given on at least fifteen (15) days notice.
39.1.3 Prior to the annual PFT firefighters they shall undergo a Physical Medical Examination (PME) to evaluate health status.
39.2 If a firefighter is taking sick leave or vacation while the PME and PFT are being conducted, they will be excused from the test for that date. The PME and PFT shall be scheduled for a subsequent date.
39.3 If a Firefighter is required to take the PME or PFT while off-duty, they will be compensated in accordance with Article 17, Overtime.
39.4 The purpose of this Fitness Program is to ensure that all personnel will maintain a state of health and physical conditioning that will permit them to carry out basic fire suppression and rescue tasks efficiently and without undue risk to themselves and others.
39.5 The PME will include: Basic Exam, Exertion Test, Chemical Panel and Screening (Blood Workup) TB Test and update of Tetanus shot if necessary.
39.5.1 The Town will provide an optional, as needed, chest x-ray for each firefighter once every five (5) years.
39.5.2 The Chief shall develop a rotating PME schedule for all firefighters.
39.5.3 Town will pay for an HIV screen, with confidentiality of results as per all applicable laws, and Hepatitis vaccinations at the request of the firefighter.
39.5.4 The PME shall be performed by the Town’s health care provider at no cost to the firefighter.
39.5.4.1 Any additional follow-up work needed for diagnosis purposes as recommended by the Town’s health care provider during the PME, will be paid for by the Town.
39.5.5 Should a firefighter, after the PME, be considered unfit to participate in the PFT, they will receive recommendations, designed to help them attain a fitness level, which will enable them to participate safely in the program.
39.6 Firefighters shall not take the PFT while under a Physician’s care and shall only do so after approval of the Physician.
39.7 Specific requirements for the PFT are contained in the Town of Brunswick Fire Department Fitness Manual. The PFT will consist of the following components.
39.7.1 Cardiovascular endurance. Choose one (pass/fail)
a. One mile walk test b. Step test c. 1.5 mile run
HEALTH AND FITNESS. 18.1 In addition to other types of leave you will be entitled to one (1) "wellness day" per year. This day is ideally planned in advance and taken at a time that will not adversely affect the business; however, the days' leave will not be unreasonably withheld if taken as an additional day of unplanned sick leave.
HEALTH AND FITNESS. A. All employees will be required to submit to a physical examination every eighteen (18) months by a licensed medical physician selected by the Employer. The Employer shall be responsible for the cost of the physical examination.
B. All employees must be fit-tested for a Self-Contained Breathing Apparatus (S.C.B.A.) and have no restriction on using the department S.C.B.A. Respirator. The Fire Department will administer fit-test or, at its option, pay to have the testing done.
C. Department required physicals will be scheduled and completed while on-duty when possible. Department physicals may be scheduled while off-duty based on the availability of appointment dates from physician. When scheduled while off-duty, the Fire Fighter will receive overtime compensation for the documented time period of the appointment plus travel time. All physical examinations shall be in accordance with the National Fire Protection Association (NFPA) 1582 standards applicable to members (not candidates).
D. In case of conflicting medical opinions, the Township’s physician and employee’s Physician shall choose a third Physician whose opinion will be final and conclusive.
E. Detailed physical reports will be shared only between the selected doctors and the individual patient/Fire Fighter. All HIPAA guidelines will be followed and a signed HIPPA letter will be utilized to prevent improper distribution of personal medical information. The Township and Fire Administration will only be notified that the candidate is or is not fit for duty. If the patient/Fire Fighter is deemed un-fit for duty, further testing will be conducted as outlined in Paragraph C.
