Vision Service Plan (VSP) Sample Clauses

Vision Service Plan (VSP). Effective with May 2011 work hours, the Employer shall pay the necessary additional contribution to California Service Employees Trust Fund to add the Vision Service Plan (VSP) to the existing medical, prescription dental and life insurance plan for employees working in Areas 1 and 2. The eligibility for said plan shall be the same as set forth for the Health and Welfare Medical Plan. The Employer agrees to pay monthly the contribution required as determined by the Board of Trustees for this benefit effective with May 2011 work hours for each eligible employee. The 2009 monthly cost is $16.00 per month per member, subject to the MOB provisions of this Article XXXIII.

  • In-Service Programs The parties to this collective agreement recognize the value of in-service education both to the employee and the Employer.

  • Vision Care Plan The County agrees to provide a Vision Care Plan for all employees and dependents. The Plan will be the Vision Service Plan - Plan A with benefits at 12/12/24 month intervals and with twenty dollar ($20.00) deductible for examinations and twenty dollar ($20.00) deductible for materials.   • Special Service networks The following services must be received from special service network providers in order to be covered. All terms and conditions outlined in the Summary of Benefits apply.

  • Oregon Public Service Retirement Plan Pension Program Members For purposes of this Section 2, “employee” means an employee who is employed by the State on or after August 29, 2003 and who is not eligible to receive benefits under ORS Chapter 238 for service with the State pursuant to Section 2 of Chapter 733, Oregon Laws 2003.
