Xxxx Whole Health Clinic Health Assessment Incentive. Employees enrolled on the Plan and who completes an Annual Whole Health Evaluation (AWHE) at the Xxxx Care Center may be qualified to earn an incentive. Xxxx will report to the Municipality the names of eligible employees who have completed the AWHE. The Municipality will not have access to employee health information. The contribution is not eligible for PERS and will be subject to applicable taxes and withholdings. The MOA may end this incentive at any time.
Xxxx Whole Health Clinic Health Assessment Incentive. Employees enrolled on the Plan and who complete a four (4) step Annual Whole Health Evaluation at the Xxxx Health Clinic may be qualified to earn a $200 incentive contribution to their paycheck. The annual Whole Health Evaluation consists of the following four (4) steps: • Biometric Screening, • Health Assessment, • Provider Wellness Exam, and • Introduction to Coaching Xxxx will report to the Municipality the names of eligible employees who have completed the four-step evaluation. The Municipality will not have access to employee health information. The Municipality agrees to add the contribution to the employee’s paycheck within forty- five (45) days of the Municipality being notified that all four (4) parts of the evaluation have been completed. The contribution is not eligible for PERS, and will be subject to applicable taxes and withholdings.

  • OCCUPATIONAL HEALTH & SAFETY (a) It is a mutual interest of the parties to promote health and safety in workplaces and to prevent and reduce the occurrence of workplace injuries and occupational diseases.
