Xxxxxxxxxxx from Bank. A tenured member of the Federation Bank who has exhausted their accumulated personal/sick leave allowance, who has been absent for at least five (5) unpaid working days, may request withdrawals from the Federation Bank. A physician’s statement shall be included with each request. An employee may be granted a maximum of one hundred sixty-five (165) consecutive days of Federation Bank leave per school year if there are sufficient days available in the Bank. Each application for Federation Bank leave may not recess periods shall not be considered an interruption in an employee’s use of consecutive Federation Bank days for the same illness or condition; however, these days shall not count toward the one-hundred sixty-five (165) consecutive Federation Bank days under this section. Each member shall have a maximum benefit of three hundred fifty (350) days. After meeting the five (5) days deductible on one (1) occasion an employee will not be subject to the deductible again during that school year.

Xxxxxxxxxxx from Bank. If an eligible member desires to apply for Food and Nutrition Services Sick Bank days from the Food and Nutrition Services Sick Bank for personal use, said member shall have exhausted all individual accumulated sick leave/personal leave, and shall have contributed to the Bank for that year. In addition, a member must be absent for five (5) or more consecutive days and not receive pay for those five (5) days before eligibility occurs. After being subject to the five (5) day deductible on one occasion, a member will not be subject to the deductible again during that school year. An eligible member of the Food and Nutrition Services Sick Bank who has exhausted his/her accumulated personal/sick leave allowance, who has been absent for at least five (5) consecutive unpaid working days, may request withdrawals from the Food and Nutrition Services Sick Bank. A detailed physician’s statement shall be included with each request. A member may be granted a maximum of ninety (90) days of Food and Nutrition Services Sick Bank leave per school year if there are sufficient days available in the Bank. Each application for Food and Nutrition Services Sick Bank leave may not exceed ten (10) days of Food and Nutrition Services Sick Bank days at a time. Holidays and recess periods shall not be considered an interruption in a member’s use of consecutive Food and Nutrition Services Sick Bank days for the same illness or condition; however, these days shall not count toward the ninety (90) consecutive Food and Nutrition Services Sick Bank days under this section. Each member shall have a maximum lifetime benefit of ninety (90) d days. No Food and Nutrition Sick Bank days will be granted for absence due to injuries at work which qualify the individual for workers’ compensation.
