Change Form definition

Change Form means such hard copy and/or electronic form as may be provided by the Committee to permit an Eligible Employee to change certain Distribution Elections in accordance with Section 7.1(c), herein, applicable law and rules promulgated under the Code.
Change Form means the document or documents prescribed by the Plan Administrator and pursuant to which a Participant may change elections made on an Enrollment Form.

  • Application Form means any application form to be completed by subscribers for Shares as prescribed by the Company from time to time.
