Financial Assistance Policy definition

Financial Assistance Policy. FAP")" application" means the information and accompanying documentation that a patient submits to apply for financial assistance under Lifespan's FAP. Lifespan may obtain information from a patient in writing or orally (or a combination of both).
Financial Assistance Policy or “Policy” means the terms and conditions found in this document.
Financial Assistance Policy means a policy that meets the requirements of section 501(r) of the Internal Revenue Code and implementing regulations.
