Aim of the Service. To provide a community needle and syringe exchange scheme for injecting drug users.
Aim of the Service. 2.1 To ensure that patients requiring prescribed sharps can return them in an approved sharps container to a community pharmacy for safe disposal.

The breastfeeding support service will deliver an evidence base service that will provide support to pregnant women and new mothers to enable them to breastfeed their babies and thus take advantage of the health benefits breastfeeding affords to both mother and baby in the short, medium and long term.

The aim of the Involve Kent is: To provide volunteer services in the Borough excluding the Tonbridge, Hadlow and Hildenborough area following the six core functions of volunteering infrastructure as set out by NCVO: o Brokerage – matching groups and individuals with appropriate volunteering opportunities o Marketing Volunteering – promoting and encouraging local interest in volunteering and community activity o Good Practice Development – promoting good practice in working with volunteers to all volunteering organisations o Developing Opportunities – working in partnership with statutory, voluntary and private sector organisations, community and specialist groups to develop local volunteering opportunities. o Policy response and campaigning – identifying strategic proposals and legislation that impacts on volunteering. Participating in Campaigns and representing the voice of volunteers at a strategic level o Strategic development of volunteering – providing expert input to strategic thinking and planning locally, regionally and nationally
Aim of the Service. 2.1 To enable any suitable patients to be prescribed free Emergency Hormonal Contraception (EHC) under a Patient Group Direction (PGD) in community pharmacies in Cheshire West and Chester. This will contribute to better reproductive health and wellbeing outcomes for women in West Cheshire who require the service. For the evidence base see appendix 1.
Aim of the Service. 2.1 To enable any suitable patients to be prescribed free Emergency Hormonal Contraception under a Patient Group Direction (PGD) in community pharmacies in Cheshire West and Xxxxxxx. This will contribute to better reproductive health and wellbeing outcomes for Cheshire West and Xxxxxxx women who require the service.
Aim of the Service. The principal aim of the service is to help elderly persons carry out small scale works of repair and maintenance to their homes to enable them to live more independently, improve their quality of life and reduce the risk of personal injury. It will complement other agencies such as Daventry Volunteer Centre and schemes run by Helped the Aged etc. The scheme will incorporate home security works and may well be a source of referrals for additional works to Care and Repair.
Aim of the Service. To identify individuals at a high risk of CVD, predictive risk registers are being compiled in all GP surgeries in Manchester. The success of these registers in identifying and managing individuals at risk of developing CVD is dependent upon the availability of accurate and up to date information about risk factors. There are gaps in the patient information currently available and a number of individuals have insufficient information recorded to enable a calculation of risk to be made. Therefore NHS Manchester wishes to commission a targeted pilot vascular checks programme to be delivered across the city in order to supplement the information held on GP practice systems. The Department of Health have stipulated that PCTs will begin the roll out of a National Vascular Checks Programme for adults aged between 40-74 years from April 2005 and for this to be completed by 2012. Repeat checks will then be offered every five years. A national call and recall system is planned within 5 years. This service is intended to be a pilot prior to the roll out of a wider scale vascular checks service in line with National guidance. The aims of the Targeted Vascular Checks Pilot Programme are to: • identify asymptomatic individuals at high risk of developing CVD, and individuals with no prior diagnosis of CVD but who demonstrate symptoms of disease • ensure that such individuals are entered into the appropriate care pathway • undertake the appropriate anthropometric measurements to facilitate the compilation of predictive CVD risk registers in General Practice.
Aim of the Service. The aims of the Targeted weight assessment Pilot Programme are to accurately weigh individuals at high body weight [in excess of 200kg] and to ensure that such individuals are entered into the appropriate care pathway. There are gaps in the patient information currently available and a number of individuals have insufficient information recorded to enable an assessment of suitability for onward referral for bariatric surgery to be made. Therefore NHS Manchester wishes to commission a targeted pilot weight check programme to be delivered across the city in order to supplement the information held on GP practice systems. This service is intended to be in support of the Specialised Weight Management Service (SWMS) The aims of the Targeted Weight Check Pilot Programme are: • To offer targeted weighing in an easily accessible community setting at appropriate times of the day. • To accurately test and record specific anthropometric and lifestyle information. • To communicate this weight to the individual if they require the data. • To accurately and promptly report information to General Practice. .
