Report Information. Monthly Performance Report. NAVITAIRE will submit a Monthly Performance Report by the tenth business day of the subsequent month following the Reporting Period to the Customer Account Liaison. The report will contain the monthly indicator of Service Levels statistics and will be transmitted via email unless otherwise requested by Customer. The report will also summarize all Interrupted Service Reports for the Reporting Period.

This information must include: dates, times, and locations of survey operations; time of observation, location and weather; details of marine mammal sightings (e.g., species, numbers, behavior); and details of any observed Taking (e.g., behavioral disturbances or injury/mortality). U.S. DEPARTMENT OF THE INTERIOR BUREAU OF OCEAN ENERGY MANAGEMENT The Lessee must perform surveys for the identification of archaeological resources in support of plan (i.e., Site Assessment Plan [SAP] and/or Construction and Operations Plan [COP])) submittal as described below. Surveys for the identification of archaeological resources performed in support of plan submittal are hereinafter referred to as “archaeological surveys.” , In accordance with the reporting requirements described in this Appendix, the Lessee must prepare an archaeological-resource assessment report, which presents the results of the archaeological surveys . The Lessee must submit an archaeological- resource assessment report to the Lessor with any plan required to be submitted by the Lessee for activities associated with this lease.

Report Information. Lessee must ensure that the protected species observer record all observations of protected species using standard marine mammal observer data collection protocols. The list of required data elements for these reports is provided in Appendix B.

Reports shall be prepared and transmitted in accordance with the Business and Operations Plan, and also contain the following information: on a daily basis, number of late and missed trips and the causes; on a monthly basis, number of late and missed trips and the causes and passenger counts by route and by fare type (i.e. adult, child who rode free of charge, FlyAway Employee Monthly Pass, Metrolink Monthly Pass, Metrolink TVM, Metro TAP, Metro EZ transit pass, and ADA), trip, day, month, departure times and travel times, revenue by route and by source (i.e. credit, debit, promise to pay, online, FlyAway Employee Monthly Pass, Metrolink Monthly Pass, Metrolink TVM, Metro TAP, and Metro EZ transit pass); on an annual basis, fleet information by route including vehicle identification numbers, equipment used, engine type, and fuel used. The form of these records shall be subject to the approval of the Chief Executive Officer.”
Report Information. The Treasurer shall prepare a report to the City Council not less than semi-annually which is available each year within 60 days following December 31st and June 30th. The semi-annual report shall be presented at a subsequent regularly scheduled City Council Meeting. The report shall be inclusive of a monthly listing of investment transactions. At a minimum the report shall include the following (Revised 9-18-2012):
a) Type of Investment
b) Issuer
c) Date of Maturity
d) Par and dollar amount invested
e) Current Market Value as of the date of the report
f) Source of the market value information
g) A list of investment transactions.
h) A statement of compliance with the investment policy
i) A statement as to the ability of the City to meet its expenditure requirements for the next six months In addition, the City Treasurer will submit a monthly transaction report to the City Council.
Report Information. The following schedules set forth the specific data elements that shall be provided for each UPR submitted in accordance with the terms of this Agreement (with each data element representing a column heading on a report). Prior to the first submission of each of the below schedules, the Company and Verus will meet in order to make any changes to the column headings that are operationally necessary and mutually agreeable.
