Average Length of Stay. The length of stay at the Restpadd Health Corp. is planned to meet the acute psychiatric needs of the patients referred to the program. Restpadd Health Corp. will accept both voluntary and involuntary patients who meet the admission criteria for the program. It is expected that many of the patients referred to Restpadd Health Corp. will be on an involuntary hold, which is limited to 72 hours. These patients may receive treatment beyond that time frame if they meet continued medical necessity. To continue treatment, a patient must either agree to be treated on a voluntary basis or must be mandated to continue on an involuntary basis by the proper legal authority. It is anticipated that the average length of stay at the PHF will be between 3 to 5 days. Stays of less than three days, or longer than five days, will be dependent on the individual needs of the patient. In all cases, individual care will be coordinated with the County MHP. Discharge planning and Aftercare will be coordinated with the patient’s referring agency/caseworker to ensure post-discharge placement, medication support and social, vocational and educational services as appropriate.
Average Length of Stay. The average length of stay of Medicare inpatients at each Facility shall have been greater than 25 days for the first five (5) consecutive months within the six (6) month period immediately prior to the Closing Date. Such calculation shall include Medicare Advantage inpatients if allowed by CMS.

Stays of less than three days, or longer than five days, will be dependent on the individual needs of the patient. In all cases, individual care will be coordinated with the County Contract Administrator. Discharge planning and aftercare will be coordinated with the patient’s referring agency/caseworker to ensure post-discharge placement, medication support, and social, vocational, and educational services as appropriate. CONTRACTOR is responsible for coordinating the logistics for certification review, involuntary medication, and writ hearings. COUNTY is not responsible for providing staff for advocacy during these hearings or associated costs for these hearings.
Average Length of Stay. The average length of stay for enrollees ages 21-64 who received treatment in an IMD during CY 2017 was approximately 6 days. The minimum length of stay was 1 day and the maximum length of stay was 24 days. Note that as described above, costs were identified and removed from the encounter data for members with an IMD stay of more than 15 days in a month.

These patients may receive treatment beyond that time frame if they meet continued medical necessity and they either (a) agree to be treated on a voluntary basis or (b) are mandated to continue treatment on an involuntary basis by the proper legal authority. The average length of stay at either facility is between 8-11 days. Stays of less than five (5) days or longer than seven (7) days are dependent on assessment of the patient’s needs. All care provided, including discharge planning and aftercare is coordinated with the patient, the patient’s responsible party and the patient’s referring agency to ensure continuity of care, and appropriate discharge placement.
5.1. CONTRACTOR will engage in discharge planning beginning at intake for each client served under this agreement. Discharge planning will include regular reassessment of client functioning, attainment of goals, determination of treatment needs and establishment of discharge goals.
5.2. When possible, discharge will include treatment at a lower level of care or intensity appropriate to client’s needs and provision of additional referrals to community resources for client to utilize after discharge.
5.3. The COUNTY’s designated discharge planner will assume primary responsibility for coordination or linkage to other programs or resources within Placer County, collaborating with outside counties, transportation, and assisting with necessary transfer of discharge paperwork. Aftercare plans are completed that address the patient’s need for follow-up treatment, housing, and socialization. Each discharge record shall have a discharge summary that includes a recapitulation of the patient’s stay and recommendations regarding follow-up and aftercare from each appropriate discipline. It also includes a brief summary of the patient’s condition upon discharge.
