FACILITY SHALL. A. Provide facilities for learning experiences for Human Services students designated by the UNIVERSITY. The experience for each student shall cover such period of time as will be specified by the UNIVERSITY. B. Maintain facilities in conformance with University standards. C. Permit designated students and staff of UNIVERSITY to use all services of the FACILITY herein contracted for. The level of services and the number of students involved shall be determined by mutual agreement between the parties. D. Nominate staff members to serve as volunteer preceptors. The final selection of the preceptors shall be made by mutual consent between the FACILITY and the Faculty Coordinator. E. Permit University faculty and staff to participate as their time may permit in the learning experience of the students while at Facility. F. Be responsible for all instruction and evaluation of student performance required to meet the course objectives given by University at the FACILITY site to the students so designated.

FACILITY SHALL accept a mutually agreed upon number of Students whom District has selected for an Experience period;
FACILITY SHALL. A. Permit each student designated by the University to receive social work field education experience at the Facility and shall permit such students and University social work instructors reasonable access to appropriate social work facilities for such social work field education experience. B. Maintain the facilities and provide opportunities in such a manner that the minimum essentials (adequate supervision, safe environment and access to facility and supplies) for an approved field education experience shall be met at all times. C. Assure that staff is adequate in number and quality to ensure safe and continuous client services to individuals. Facility shall maintain sole responsibility and accountability for services to children and families. D. Permit the Facility's social work director and other designated personnel to attend University social work field meetings, or any committee thereof, to coordinate the field education experience program provided for under this Agreement. E. Have the right to refuse participation to any University student who is not participating satisfactorily in the program. In the event Facility determines a student is not satisfactorily participating in the program, Facility shall consult with University regarding the reasons for denying participation of such student. F. Notify University social work field education program of any change in the Facility's social work director/management appointments. G. Coordinate emergency first aid or medical treatment if a student suffers an injury or illness during the course of a student’s field education experience. H. Assist University in verifying the student’s attendance and participation in the program, meeting requirements for awarding of academic credit.

Permit students, designated by the University to receive Speech Pathology fieldwork experience at Facility, and shall furnish and permit such students, and University instructors reasonable access to a variety of professional opportunities within the scope of services offered by the Facility for such internship experience. B. Maintain the facilities and provide opportunities in such a manner that the minimum essentials (adequate supervision, safe environment and access to facility and supplies) for an approved fieldwork experience shall be met at all times. C. Permit and encourage members of the Facility's staff to participate in the instructional phase of the fieldwork experience. D. Permit the Facility's Director or other designated personnel to attend meetings of the University's Communicative Disorders Department Faculty, or any committee thereof, to coordinate the internship experience of the program provided for under this agreement. E. Have the right to refuse participation to any University student who is not participating satisfactorily in the program. In the event Facility determines a student is not satisfactorily participating in the program, Facility shall consult with University regarding the reasons for denying participation of such student. F. Notify the University's instructors, of any change in the professional credential status of the Facility's Speech Therapists (where applicable).
FACILITY SHALL. A. Provide clinical facilities for learning experiences for nursing students designated by the UNIVERSITY. The clinical experience for each student shall cover such period of time as will be specified by the UNIVERSITY. B. Maintain clinical facilities in conformance with standards of the State Board of Nurse Examiners and permit inspection of its clinical facilities upon request by the Council of Baccalaureate and Higher Degree Programs of the National League for Nursing and by authorized representatives of the UNIVERSITY. C. Permit designated students and staff of UNIVERSITY to use all services of the FACILITY herein contracted for. The level of services and the number of students involved shall be determined by mutual agreement between the parties. D. Nominate staff members to serve as clinical preceptors. The final selection of the preceptors shall be made by mutual consent between the FACILITY and the Faculty Coordinator. FACILITY SHALL a. Provide clinical facilities for learning experiences for Master of Social Work students designated by the UNIVERSITY. The experience for each student shall cover such period of time as will be specified by the UNIVERSITY. b. The FACILITY shall recommend members of its staff, qualified and approved within the UNIVERSITY’S criteria, to act as Fieldwork Instructors (“Instructors”) for students placed for training. These instructors shall be afforded sufficient release time to instruct and supervise the student’s work at the FACILITY in accordance with the educational objectives, learning experiences and performance expectations established by the UNIVERSITY and agreed to by the FACILITY. c. The FACILITY shall provide facilities, staff, materials, and other resources necessary to meet the FACILITY’S educational commitment. d. The FACILITY shall accept students enrolled in the UNIVERSITY for a Practicum in graduate social work education at times, in numbers and at such locations of the FACILITY as shall be agreed upon by both parties. e. The FACILITY shall not use students to replace its regular staff and shall not require the students to render services except as they are identified for their learning value as part of an agreed upon educational purposes. f. The FACILITY shall not compensate students for their services unless otherwise and, previously agreed to by the UNIVERSITY. g. The FACILITY may request that the UNIVERSITY remove any student whose performance the FACILITY deems unfit to meet the demands of its service program or whose conduct otherwise interferes with its staff relationships or primary mission. h. FACILITY is aware of and informed about the hazards currently known to be associated with the novel coronavirus referred to as “COVID-19”. FACILITY is familiar with and informed about the Centers for Disease Control and Prevention (“CDC”) current guidelines regarding COVID-19 as well as applicable federal, state, and local governmental directives regarding COVID-19. FACILITY, to the best of its knowledge and belief, is in compliance with those current CDC guidelines and applicable governmental directives. If the current CDC guidelines or applicable government directives are modified, changed or updated, FACILITY will take steps to comply with the modified, changed or updated guidelines or directives. B. It is expected, in accordance with the presently established values and goals of the University and MS Clinical Program, that placement facilities provide the following: 1. A fieldwork setting that offers the opportunity to serve various client populations. This includes opportunities to work with clients who are oppressed, marginalized, and/or disenfranchised by virtue of their ethnicity, sexual orientation, socioeconomic status, immigration experience, age, and/or disability; 2. A setting that offers opportunities for students to examine their own commitment to these populations; 3. A setting that offers students opportunities to test, modify and integrate the ideas, concepts and values of the MS Clinical Program curriculum into their emerging professional selves (e.g., evidence-based approaches to psychotherapy); and 4. A setting that provides students with an opportunity to develop skills and practical knowledge by working with clinical populations under the guidance of a skilled California licensed supervisor. C. It is required that each Placement Facility shall: 1. Provide a learning experience that entails a commitment to service compatible with the values and ethics of the MFT and LPCC profession as outlined by the Board of Behavioral Sciences and the California Association of Marriage and Family Therapists; 2. Provide a qualified clinical fieldwork supervisor with appropriate and active license as defined by the California Board of Behavioral Sciences for master’s level trainees (e.g., licensed psychologist, LMFT, LPC, or LCSW). Assessment of qualifications will take into account professional education, commitment to the values of the profession, competence in practice, and interest and competence in teaching and supervising; 3. Provide the required number of clinical supervision hours as specified by the California Board of Behavioral Sciences for masters level MFT and LPC trainees in both individual format and group format; 4. FACILITY SHALL. A. Accept Nurses in accordance with the requirements of the FACILITY’s applicable clinical affiliation program and the ability of FACILITY to provide the required learning experience and training as stated in the program objectives. The amount of clock hours and the rate of completion shall be defined and agreed upon by the Parties as set forth in Exhibit C, B. Provide Nurses with appropriate orientation as to FACILITY’s general hospital and programmatic policies, procedures, protocols, and practices, as well as applicable standards by appropriate accrediting agencies. C. Consider the Nurses as members of the health care team, and allow them to provide designated patient care services under supervision of a preceptor employed by FACILITY, in keeping with physician’s orders and FACILITY’s policies, procedures, protocols, and guidelines. D. Encourage FACILITY staff to cooperate with the HOSPITAL in the planning and learning activities related to the Nurses. Such participation shall be discretionary with individual members of the FACILITY staff, and shall not be permitted by FACILITY if participation interferes with duties assigned to members of the FACILITY staff. E. Accept that the status of the Nurse is one of a learner, and that Nurses shall not replace, or assume the role of, members of FACILITY staff. F. As appropriate and as allowed by applicable laws, regulations and this Agreement, provide, make available, or permit use of: 1. Patient care and service facilities; 2. Supplies and equipment required for patient use; 3. Unreserved rooms or areas where Nurses may hold discussions and receive clinical instruction; and, 4. Relevant patient records, subject to all applicable safeguards of patient confidentiality, in accordance with established hospital policies and procedures. G. Notify HOSPITAL, within five working days, should any situation or problem arise which impedes the educational process or threatens completion of the instructional period for the Nurse. H. Refuse access to its clinical areas to any Nurse who does not meet FACILITY's standard for safety, health or appropriate workplace and ethical behavior. Notice of the action set forth in this paragraph shall be given to HOSPITAL within three working days after such action. I.
