Care Team. The Parties’ subcontract shall require that the CP take the lead on forming and coordinating a Care Team for each Enrollee, as described in Section 2.6.C.4 of the Contract. In addition, the CP shall ensure: 1. That the Care Team meets at least twice within a 12-month period, and 2. That a representative from the care team attends any multidisciplinary team meetings hosted by the Contractor, clinical staff, hospitals and/or other stakeholders to review high-risk Members, if applicable;

Care Team a multidisciplinary team responsible for coordinating certain aspects of a member’s care, as further described in Section 2.4.C.
Care Team. A team of professionals serving Members identified as requiring Conflict Free Case Management (CFCM), Care Management (CM), or Complex Case Management (CCM) that collaborate, in person and/or through other means, with Members to develop and implement an Individualized Care Plan (ICP) that meets Members’ medical, behavioral, Long-Term Services and Supports (LTSS), and social needs.
Care Team. The Campus Assessment, Response, and Evaluation (CARE) Team is a multi- disciplinary group who meets regularly to assess, evaluate, and respond to reports submitted concerning an enrolled student’s behavior and/or the behavior of another campus community member. Typical reports pertain to students who exhibit signs of distress, show a disturbance, and/or that present a danger.
Care Team. The Care Team advisors (maximum of two per building) will have a duty-free period.

Clinical Care Manager – as used in Appendix G a licensed Registered Nurse or other individual, employed by the Contractor or an Enrollee’s PCP and licensed to provide clinical care management, including intensive monitoring, follow-up, and care coordination, and clinical management of high-risk Enrollees, as further specified by EOHHS. Clinical Quality Measures – clinical information from Enrollees’ medical records used to determine the overall quality of care received by Enrollees or Members. Clinical Quality Measures are a subset of Quality Measures and are set forth in Appendix B. Material Subcontractors with a Member who is not enrolled in the Contractor’s plan that EOHHS can reasonably interpret as influencing the Member to enroll in the Contractor’s plan or either not to enroll in, or to disenroll from, another Accountable Care Organization, MCO, or the PCC Plan. Cold-call Marketing shall not include any personal contact between a Provider and a Member who is a prospective, current or former patient of that Provider regarding the provisions, terms or requirements of MassHealth as they relate to the treatment needs of that particular member. Related to Care Team

  • The Joint Committee (a) shall be composed of representatives of the Governments of the Parties; and (b) may establish and delegate its responsibilities to Sub-Committees.
