Co-Pay. Effective July 1, 1999, any Employee who continues to participate in the “Traditional State Health Benefit Plan shall pay the difference between the traditional plan and the NJ Plus PPO plan. Deductions will be made from the Employee’s paycheck on a biweekly basis.

Co-Pay. Dakota Vascular requests that co-pays are paid at the time of registration. Copays will be collected based on the amount listed on the insurance card or the insurance carriers website. Co-pays can be paid by check, cash, or credit card. Credit cards accepted include Discover, Visa, and Mastercard. Post-dated checks will not be accepted. Dakota Vascular requires that all cash pay or self-pay services are paid in full at the time of service. If the patient or responsible party are not able to pay in full, prior arrangements must be made.
Co-Pay. All employees hired after January 1, 1994 will contribute up to 15% of the cost of the medical benefit insurance premium. However, such co-payment shall not exceed 5% of the individual employee's base salary. For 2017, employees shall continue to contribute towards their premiums at Tier 4 of Chapter 78. For 2018, employees shall contribute towards their premiums at Tier 3 of Chapter 78. For 2019 and 2020, employees shall contribute towards their premiums at Tier 2 of Chapter 78.
Co-Pay. Applicable co-payments for in- and out-of-network services under the State Health Plan are set forth in Appendix J. Effective October 1, 2008, there will be a $15 co-pay for an office visit, and a $50 co-pay for emergency room visits if the patient is not admitted to the hospital. All other applicable co-payments for in-network and out-of-network services under the State Health Plan are set forth in Appendix X.

  • Reimbursement Payments The Department shall, to the extent funds are available, reimburse the Grantee for eligible claims presented for payment if the Department determines the requirements for reimbursement have been met. To be eligible for reimbursement, all costs must be reasonable, allowable, and properly allocated for support of the xxxxxx care program. A direct or enhanced administrative claim is not eligible for reimbursement if the basis of the claim has funding from any other federal source. 4.3.2. JJAEP Program (Grant "P"). Grantees eligible for reimbursements under Xxxxx X shall receive a share of the initial $1,500,000 distribution based on each Grantee's share of the total juvenile population for each school year for the current contract period. Additional funds will be distributed at a rate not to exceed $96 per eligible student attendance day for students who are required to be expelled pursuant to Chapter 37 of the Texas Education Code and who meet the Targeted Grant requirements. The Grantee will not be able to receive the additional funds until the initial amount allocated is earned at the rate of $86 per eligible student attendance day. Payments to the Grantee by the Department shall be limited to no more than 180 days of operation during each regular school year for the current contract period.
