Co-Pay. Applicable co-payments for in- and out-of-network services under the State Health Plan are set forth in Appendix J. Effective October 1, 2008, there will be a $15 co-pay for an office visit, and a $50 co-pay for emergency room visits if the patient is not admitted to the hospital. All other applicable co-payments for in-network and out-of-network services under the State Health Plan are set forth in Appendix X.

Co-Pay. The percentage of the covered allowable charge for which you are responsible per Pet and which is not reimbursable under this policy. Co-pay shall be applied after any deductible.
Co-Pay. Dakota Vascular requests that co-pays are paid at the time of registration. Copays will be collected based on the amount listed on the insurance card or the insurance carriers website. Co-pays can be paid by check, cash, or credit card. Credit cards accepted include Discover, Visa, and Mastercard. Post-dated checks will not be accepted. Cash Pay Services Dakota Vascular requires that all cash pay or self-pay services are paid in full at the time of service. If the patient or responsible party are not able to pay in full, prior arrangements must be made.
Co-Pay. All employees hired after January 1, 1994 will contribute up to 15% of the cost of the medical benefit insurance premium. However, such co-payment shall not exceed 5% of the individual employee's base salary. For 2017, employees shall continue to contribute towards their premiums at Tier 4 of Chapter 78.   • Co-Payments Effective January 1, 2020, the State Dental Plan will cover allowable charges for the following services subject to the co- payments and coverage limits stated. Higher out-of-pocket costs apply to services obtained from dental care providers not in the State Dental Plan network. Services provided through the State Dental Plan are subject to the State Dental Plan's managed care procedures and principles, including standards of dental necessity and appropriate practice. The plan shall cover general cleaning two (2) times per year and special cleanings (root or deep cleaning) as prescribed by the dentist.

  • Shift Pay Each full-time employee scheduled to work on any shift commencing after 9:00 a.m. shall be paid a shift premium of seventy- five cents ($0.75) per hour for all hours worked by him/her on that shift and overtime hours worked in continuity with such shift. Shift premiums payable under this provision shall not be included as part of the employee’s regular pay for purposes of computing overtime pay.

  • Premium Pay “Premium Pay” is a special pay rate for working during times that are less desirable, such as weekends, holidays or late shifts. The City will not pay the Consultant Premium Pay.

  • Lump Sum Payments If, during the Employment Period, the Company terminates the Executive's employment other than for Cause, or the Executive terminates employment for Good Reason, the Company shall pay to the Executive the following amounts:

  • Xxxx Pay We will process xxxx payment transfer requests only to those creditors the Credit Union has designated in the User Instructions and such creditors as you authorize and for whom the Credit Union has the proper vendor code number. We will not process any xxxx payment transfer if the required transaction information is incomplete. We will withdraw the designated funds from your share draft account for xxxx payment transfer by the designated cutoff time on the date you schedule for payment.   • Retirement Pay Any teacher with ten (10) years consecutive teaching experience in the Park Hill School District immediately prior to retirement from PSRS without an age reduction for early retirement, shall receive upon retirement from the Park Hill School District a terminal amount based upon the following formula: (Notation, the teacher must make application to PSRS for retirement and begin drawing from PSRS on the first available month following retirement). Years of service to the Park Hill School District to be divided by ten (10) and multiplied by one-ninth (1/9) of the last completed contract. Retirement notification after December 15 for the current academic year will result in a reduction of $1,000.00 from the total under Article 36. In the event of a sudden severe illness of the teacher, teacher’s legally recognized spouse, and/or child, the transfer of a legally recognized spouse, or being called into active military duty may be cause for the District not to impose the late notification reduction of $1,000.00. A teacher who otherwise qualifies for payment under Article 36 and dies while currently classified as an active employee will receive such payment.

  • Holiday Premium Pay A Nurse working on a recognized Holiday is entitled to the following compensation for any hours worked on the calendar date of the recognized Holiday:

  • You must also pay (a) Late payment charge a monthly late payment charge at a rate(s) determined by us and notified to you from time to time if we do not receive your full payment of the minimum payment amount specified in the statement of account on or before the payment due date; and

  • Special Pay Special payment, including standby, overtime, premium, and other special payments, shall be calculated in accordance with the applicable provisions of this Agreement.

  • Holiday Pay A. On each of the holidays designated above, each full-time employee scheduled to work but permitted to take the day off shall receive pay computed at the employee's basic hourly rate for the number of hours the employee was regularly scheduled to work.
