Cohorts. (1) Cohort 1 and Cohort 2. Summit Learning will provide up to four (4) years of Professional Development support, inclusive of any and all prior years of support, through summer trainings (each, a “Summer Training”) and regional convenings (each, a “Regional Convening”) to partner schools that began student use of the Program during the 2015-16 school year (“Cohort 1”) and the 2016-17 school year (“Cohort 2”). (2) Cohort 3. Summit Learning will provide Summer Trainings and Regional Convenings for up to three (3) years, inclusive of any and all prior years of support, through to partner schools that began student use of the Program during the 2017-18 school year; provided, however, that, Summit Learning will provide one (1) additional year of support through Summer Trainings and Regional Convenings if such partner school adds a new team of grade level teachers who teach the core classes and increases the number of students in the Program. (3) Cohort 4. Summit Learning will provide Summer Trainings and Regional Convenings for up to (3) years for partner schools that commence student use of the Program during the 2018- 19 school year (“Cohort 4”).

Cohorts. Grades 2-5 AM and PM sessions will be balanced to the extent possible, with no more than 15 students in each in-person session.
Cohorts i. Class rosters will be split into cohorts as outlined for elementary and secondary in Attachment A. Elementary, TK-8 schools and Discovery High School will follow an A/B schedule for all grades. All remaining secondary programs will follow an A/B/C schedule to maintain smaller class sizes for in-person instruction. The total number of students on a class roster will align with the collectively bargained class size language. ii. Classes would be balanced to the best of the site administrator’s ability as equal students in each cohort. General cohort size will be limited by the social distancing guidelines provided by state and local health departments. As of now that distance is six feet. iii. The cohorts that are not in-person shall engage in the class via distance learning to receive daily live instruction along with their peers who are attending class in-person. iv. The District acknowledges that some families may opt for their student to remain in Distance Learning regardless of being a member of any cohort. These students will attend class via Distance Learning to receive daily live instruction five days per week, and this will further reduce the number of students who will be present for in-person instruction.
Cohorts. The cohorts consisting of 2-3 5th grade classes each, will participate in the program during the following dates and times: Spring 2023 Dates: Cohort Max # Kids On Campus Start/End Times Field Trip Start/End Times Schools Assigned Location/Time:. The location for the Program will be held at the following locations or an alternative location(s) mutually agreed by Outfitter and Client.   • STAFF ORIENTATION 4101 The Employer shall provide an appropriate orientation program for nurses newly employed. The orientation program shall include such essential information as policies, nursing procedures, the location of supplies and equipment, fire, safety and disaster plans. Where necessary, orientation shall be provided for nurses moving to a new area of practice. 4102 The Employer shall provide a program of inservice education for nurses pertinent to patient care. 4103 The Employer shall provide, access to reference materials as is required in relation to maintaining current knowledge of general nursing care.   • Bilingual Pay Where the Employer currently pays bilingual pay or bonuses, it shall continue to do so. The Employer retains discretion to initiate bilingual pay or bonuses. The minimum bilingual bonus or hourly equivalent is $25 per pay period. The Employer may not require an employee to use bilingual skills without paying the appropriate bonus or pay. This does not apply to employees where such skills are in the classification specification.

  • Prescription Glasses This plan covers prescription glasses as follows: • Frames - one (1) collection frame per plan year; • Lenses - one (1) pair of glass or plastic collection lenses per plan year. This includes single vision, bifocal, trifocal, lenticular, and standard progressive lenses. This plan covers the following lens treatments: • UV treatment; • tint (fashion, gradient, and glass-grey); • standard plastic scratch coating; • standard polycarbonate; and • photocromatic/transitions plastic. This plan covers one (1) supply of contact lenses as follows: • conventional contact lenses - one (1) pair per plan year from a selection of • extended wear disposable lenses - up to a 6-month supply of monthly or two- week single vision spherical or toric disposable contact lenses per plan year; or • daily wear disposable lenses - up to a 3-month supply of daily single vision spherical disposable contact lenses per plan year. This plan also covers the evaluation, fitting, or follow-up care related to contact lenses. This plan covers additional contact lenses if your prescribing network provider submits a verification form, with the regular claim form, verifying that you have one of the following conditions: • anisometropia of 3D in meridian powers; • high ametropia exceeding -10D or +10D in meridian powers; • keratoconus when the member’s vision is not correctable to 20/25 in either or both eyes using standard spectacle lenses; and • vision improvement for members whose vision can be corrected two lines of improvement on the visual acuity chart when compared to the best corrected standard spectacle lenses.

  • Population The Population shall be defined as all Paid Claims during the 12-month period covered by the Claims Review.

  • Dependencies HP’s ability to deliver services will depend on Customer’s reasonable and timely cooperation and the accuracy and completeness of any information from Customer needed to deliver the services.

  • Prescriptions and bottles of these medications may be sought by individuals with chemical dependency and should be closely safeguarded. It is expected that you will take the highest possible degree of care with your medication and prescription. They should not be left where others might see or otherwise have access to them.

  • Seniority Rosters The Employer agrees to furnish the Union each January 31 and July 31 with rosters of all employees who accrue seniority, their current and prior classifications, and start date in each classification and University start date per Section 1. Employees who have accepted positions outside the bargaining unit do not accrue class seniority in the bargaining unit when employed in positions outside the bargaining unit, but retain prior seniority earned in the bargaining unit. When two (2) or more employees have the same classification seniority date, ties shall be broken by recognizing the employee with the greater University seniority first. If a tie still continues, the employee's seniority position shall be determined by lot.

  • Prescription Safety Glasses Prescription safety glasses will be furnished by the employer. The employer retains the authority to establish reasonable rules and procedures regarding frequency of issue, replacement of damaged glasses, limits on reimbursement costs and coordination with the employer's vision plan.

  • Safety Footwear 1. The State will provide employees in the positions listed in Section 3 below, and employees who are currently required to wear safety footwear by Department Work Rules, an allowance of one hundred fifty dollars ($150.00) for replacement of safety footwear. This allowance will be paid to employees on a semiannual basis, with half paid in January and half paid in July to eligible employees on the payroll at the time of the payments. Employees of Departments with work rules that provide such safety footwear will not be eligible for the safety footwear allowance. 2. Safety footwear purchased must meet ANSI standards where applicable. Requirements for the wearing of safety footwear will be in accordance with work rules published by the State. 3. Positions required to wear safety footwear:

  • Study An application for leave of absence for professional study must be supported by a written statement indicating what study or research is to be undertaken, or, if applicable, what subjects are to be studied and at what institutions.
