Data Measures. All data used in this analysis was collected by Healthy Beginnings NHN and data manager. Data is then stored in REDCap, a web-based database managing system. The REDCap data system is Health Insurance Portability and Accountability Act (HIPPA) compliant and allows for program coordinators to compile longitudinal, unidentified health histories on the children enrolled in the program. The data was collected via paper forms by the NHN and entered into the system by the data manager and Emory evaluation team interns. The program’s data is collected on an ongoing basis throughout the school year. The data used for this thesis comes from four forms including: Intake Form, NHN Visit Form, Medical Tracking Form, and SOC Exit Form (see appendix). All baseline visits for new cohort members occurred at enrollment in the beginning of the school year and was recorded via the Intake Form. The NHN Visit Form was used throughout the school year to record new health issues for individual children and their families. The Medical Tracking Form was used to follow-up with children who were reported having chronic or developmental conditions. The Exit Form was completed at the end of the year, and its purpose was to record what children are exiting Healthy Beginnings System of Care and their reason for exit. The NHN completes the Intake Form, NHN Visit Form, and Medical Tracking Form and then passes them to the Data Manager who enters the information into the REDCap database. The Exit Form is completed by the Data Manager who obtains this information from Xxxxxxx’s Procare Database.

Data Measures i. Demographic Data Aggregate data for FRL and percent Caucasian were obtained from publically available DOE data from October 7, 2014.73,74 The rationale for using 2014 data instead of 2015 was that schools are designated as “SNAP-Ed” for the 2015-2016 school year based on FRL data from 2014. Free and reduced-price lunch eligibility was reported as a continuous variable and encompassed the percentage of students eligible for FRL out of total student enrollment for each school. Race/ethnicity was self-reported by students and categorized by DOE into seven groups including Hispanic, American Indian, Asian, Black, Pacific Islander, White, and two or more races. In the current study, race/ethnicity was measured by percent Caucasian and treated as a continuous variable. Aggregate data for geography were obtained from the National Center for Educational Statistics (NCES) public school database from the 2013-2014 school year since more recent data was not available.20 Thus, schools that were created 2014 or later did not have geography data available (n=5). School geography was grouped by NCES into four categories using the following definitions: city (area in an urbanized area), suburb (area outside a major city but in an urbanized area), town (area in an urban cluster), and rural (Census-defined rural territory).20 Cohort year data was provided by HealthMPowers and was classified into four categories based on the amount of resources received from HealthMPowers: 1, 2, 3-5, and virtual. Cohort years 3-5 received similar resources and were grouped into one category, and since virtual schools were excluded from receiving any materials that are by nature designed to be in-person, these schools were classified as a unique entity in this study.

  • Data Storage Where required by applicable law, Student Data shall be stored within the United States. Upon request of the LEA, Provider will provide a list of the locations where Student Data is stored.

  • Deliverables Upon satisfactory completion of the work authorization, the Engineer shall submit the deliverables as specified in the executed work authorization to the State for review and acceptance.

  • Privacy Compliance The Provider shall comply with all applicable federal, state, and local laws, rules, and regulations pertaining to Student Data privacy and security, all as may be amended from time to time.

  • Statement of Work The Contractor shall provide the services and staff, and otherwise do all things necessary for or incidental to the performance of work, as set forth below:

  • Data Protection All personal data contained in the agreement shall be processed in accordance with Regulation (EC) No 45/2001 of the European Parliament and of the Council on the protection of individuals with regard to the processing of personal data by the EU institutions and bodies and on the free movement of such data. Such data shall be processed solely in connection with the implementation and follow-up of the agreement by the sending institution, the National Agency and the European Commission, without prejudice to the possibility of passing the data to the bodies responsible for inspection and audit in accordance with EU legislation (Court of Auditors or European Antifraud Office (XXXX)). The participant may, on written request, gain access to his personal data and correct any information that is inaccurate or incomplete. He/she should address any questions regarding the processing of his/her personal data to the sending institution and/or the National Agency. The participant may lodge a complaint against the processing of his personal data with the [national supervising body for data protection] with regard to the use of these data by the sending institution, the National Agency, or to the European Data Protection Supervisor with regard to the use of the data by the European Commission.

  • Safety Where an employee is prevented from working at the employee’s particular function as a result of unsafe conditions caused by the inclement weather, the employee may be transferred to other work in the employee’s classification on site, until the unsafe conditions are rectified. Where such alternative is not available and until the unsafe conditions are rectified, the employee shall remain on site. The employee shall be paid for such time without reduction of the employees’ inclement weather entitlement.

  • Complexity Intermediate professional level role. Provides data warehouse architectural design, development and support in multi-platform environments. Works on multiple projects as a team member and may lead projects of moderate complexity. May coach more junior technical staff.

  • De-Identified Data Provider agrees not to attempt to re-identify de-identified Student Data. De-Identified Data may be used by the Provider for those purposes allowed under FERPA and the following purposes:

  • General Requirements The Contractor hereby agrees:

  • Scope of Work For the 2022/2023 Grant Period, the Provider will maintain a victim services program that will be available to provide direct services to victims of crime who are identified by the Provider or are presented to the Provider, as specified in the Provider’s 2022/2023 Grant Application as approved by the OAG and incorporated herein by reference.
