Description of Agreement. The vendor shall furnish the supplies, equipment, and/or services, only as required by authorized NIH Buyers, during the period specified in block 17(b) of Optional Form 347 (OF 347) or block 14 of Standard Form 30 (SF 30), or until it is terminated by either party in writing or renewed by the government. BPA Calls shall be placed for those supplies, equipment, and/or services that are immediately available. If after acceptance of a BPA Call, it is realized that the item is not in stock or cannot be provided, the vendor is responsible to notify the buyer immediately.

Description of Agreement. The Master Agreement establishes the terms and conditions governing the relocation or rearrangement of SoCalGas’ facilities in physical conflict with the HSR Project. While the Master Agreement includes some of the same procedures for utility relocation in the High Speed Rail Act, it contains additional terms and conditions relating to the manner in which relocation work will be performed. SoCalGas and CHSRA will enter into project specific utility agreements that memorialize the specific scope of relocation work for any particular segment of the HSR Project, which will be performed under the general terms and conditions established in this Master Agreement. Under the Master Agreement, Section 6 stipulates that the calculation of costs for relocation of facilities includes actual and necessary cost of all engineering, environmental compliance, monitoring and mitigation, labor and transportation, and all necessary materials exclusive of any dismantled facilities used in any relocation, together with reasonable and usual indirect and overhead charges attributable to that work, and any necessary new private facility right-of-way involved in the relocation.

Description of Agreement. 1. The contractor agrees to the following terms of a blanket purchase agreement (BPA) exclusively with the Department of Veterans Affairs, VA NW Health Network (NCO20).
2. Under this agreement, the BPA holder shall provide Portable Sleep Monitoring Devices worn on the wrist and finger probe consumables associated with such devices to include service and maintenance.
3. The BPA will provide a single Item Number for a percentage discount rate off the Vendor’s complete Federal Supply Schedule (FSS) catalog. Any items associated with the sleep study devices to include consumables and maintenance of said items shall be available on schedule throughout the life of the BPA. The complete current FSS schedule catalog shall be provided to the Contracting Officer (CO) and attached to the executed BPA. As new items are added or deleted from the FSS contract the contractor shall provide an updated pricing list to the Contracting Officer within 14 days of any modifications.
4. The supplies and/or services shall be ordered by an authorized Contracting Officer during the specified period stated in the paragraph titled “Term of BPA”. This BPA is for support to NXX00 XX Healthcare Network (including geographically separated units and operating locations) only.
Description of Agreement. This Agreement consists of this Facility Operating Agreement itself and the Schedule and Exhibits attached hereto, which documents shall comprise the entire understanding and agreement between Owner and Operator regarding the subject matter hereof and supersede all other understandings, agreements and representations (oral or written) made or dated prior to the date hereof.
