Emergency Service Providers. To the extent required by the State Contract, Exhibit A, Attachment 8, Provision 31, Health Plan and/or Subcontractor is responsible for coverage and payment of emergency services and must cover and pay for emergency services regardless of whether the provider that furnishes the services has an agreement with Health Plan and/or Subcontractor.

Emergency Service Providers. Contractor will pay for Emergency Services received by a Member from non-Contractor providers. Payments to non-Contractor providers will be for the treatment of the emergency medical condition including Medically Necessary services Xxxxxx Medical Centers 95-23637 Article VI rendered to a Member until the Member's condition has stabilized sufficiently to permit discharge, or referral and transfer in accordance with instructions from the Contractor. Emergency Services will not be subject to Prior Authorization by the Contractor. The Contractor will pay for those services provided by a non-Contractor emergency department (ED) that are required to determine whether treatment of the Member's condition qualifies as an Emergency Service. At a minimum, the Contractor must reimburse the non-Contractor ED and, if applicable, its affiliated providers for Physician services at the lowest level of evaluation and management CPT (Physician's Current Procedural Terminology) codes, unless a higher level is clearly supported by documentation, and for the Facility fee and diagnostic services such as laboratory and radiology. Payment by the Contractor for properly documented claims for services rendered by a non-Contractor provider pursuant to this section will be made in accordance with Article III, Section 3.27.9, and will not exceed the lower of the following rates applicable at the time the services were rendered by the provider: A. The usual charges made to the general public by the provider. B. The maximum Fee-For-Service rates for similar services under the Medi-Cal Program. Disputed claims may be submitted to DHS for resolution under the provisions of Section 14454, W&I Code and Title 22, CCR, Sections 53620 through 53702. The Contractor agrees to abide by the findings of DHS in such cases, to promptly reimburse the non-Contractor provider within 30 days of the effective date of a decision that the Contractor is liable for payment of a claim and to provide proof of reimbursement in such form as the Director may require. Failure to reimburse the non-Contractor provider and provide proof of reimbursement to DHS within 30 days will result in liability offsets in accordance with Title 22, CCR, Section 53702.

  • Third Party Service Providers Unless otherwise prohibited by Apple in the Documentation or this Agreement, You are permitted to employ or retain a third party (“Service Provider”) to assist You in using the Apple Software and Services provided pursuant to this Agreement, including, but not limited to, engaging any such Service Provider to maintain and administer Your Applications’ servers on Your behalf, provided that any such Service Provider’s use of the Apple Software and Services or any materials associated therewith is done solely on Your behalf and only in accordance with these terms. Notwithstanding the foregoing, You may not use a Service Provider to submit an Application to the App Store or use TestFlight on Your behalf. You agree to have a binding written agreement with Your Service Provider with terms at least as restrictive and protective of Apple as those set forth herein. Any actions undertaken by any such Service Provider in relation to Your Applications or use of the Apple Software or Apple Services and/or arising out of this Agreement shall be deemed to have been taken by You, and You (in addition to the Service Provider) shall be responsible to Apple for all such actions (or any inactions). In the event of any actions or inactions by the Service Provider that would constitute a violation of this Agreement or otherwise cause any harm, Apple reserves the right to require You to cease using such Service Provider.

  • Emergency Room Services This plan covers services received in a hospital emergency room when needed to stabilize or initiate treatment in an emergency. If your condition needs immediate or urgent, but non-emergency care, contact your PCP or use an urgent care center. This plan covers bandages, crutches, canes, collars, and other supplies incidental to your treatment in the emergency room as part of our allowance for the emergency room services. Additional services provided in the emergency room such as radiology or physician consultations are covered separately from emergency room services and may require additional copayments. The amount you pay is based on the type of service being rendered. Follow-up care services, such as suture removal, fracture care or wound care, received at the emergency room will require an additional emergency room copayment. Follow- up care services can be obtained from your primary care provider or a specialist. See Dental Services in Section 3 for information regarding emergency dental care services.

  • Stock Plan Administration Service Providers The Company transfers participant data to Xxxxxx Xxxxxxx, an independent service provider based in the United States, which assists the Company with the implementation, administration and management of the Plan. In the future, the Company may select a different service provider and share the Participant’s data with another company that serves in a similar manner. The Company’s service provider will open an account for the Participant to receive and trade Shares. The Participant will be asked to agree on separate terms and data processing practices with the service provider, which is a condition to the Participant’s ability to participate in the Plan.

  • Coordination, Oversight and Monitoring of Service Providers As set forth in the Administrative Services Agreement between the Fund and CRMC, CRMC shall coordinate, monitor and oversee the activities performed by the Service Providers with which AFS contracts. AFS shall monitor Service Providers’ provision of services including the delivery of Customer account statements and all Fund-related material, including summary prospectuses and/or prospectuses, shareholder reports, and proxies.

  • Service Providers Provider shall enter into written agreements with all Service Providers performing functions pursuant to this Agreement, whereby the Service Providers agree to protect Student Data in manner no less stringent than the terms of this DPA.
