EMPLOYEE PHYSICAL EXAMINATIONS. Section 1. The City will provide a full physical examination bi-annually. The City agrees Employees may select their own physicians who shall use the “Physical Examination Form” attached hereto as “Addendum 3”. Said examination shall cover the areas included on the form in an attempt to identify any type of illness the member may have. If an Employee selects his/her own physician to perform the medical examination, the City agrees to pay $90.00 of the cost of the examination for members over the age of 40 and $70.00 of the cost of the examination for members age 40 and under.

EMPLOYEE PHYSICAL EXAMINATIONS. For reasonable cause, employees may be required to submit to physical and/or psychological examinations to establish or re-establish their physical or mental fitness to perform their work. If the Employer requires that an employee submit to an examination by an Employer-designated healthcare provider, the Employer shall pay for such examination.
EMPLOYEE PHYSICAL EXAMINATIONS. Every person, except clerical personnel, employed as a full-time member of the Police Department shall be required, as condition of their employment, prior to the effective date of their employment, to have a physical examination by a Town Physician, appointed for such purpose by the Town Manager. The cost of such physical examinations shall be borne by the Town and reports from the examining physician shall be filed with the Human Resources Director and applicable Board or Commission having jurisdiction.

  • Physical Examinations Where the Employer requires an employee to take a physical examination, doctor's fees for such examination shall be paid by the Employer. Except prior to commencement of employment and the first four (4) weeks of employment, such examinations shall be taken during the employee's working hours without loss of pay to the employee.

  • Annual Physical Examination A permanent employee shall be granted up to one day per year with pay for the purpose of a comprehensive physical examination provided that the verification of such an examination is submitted to the District.

  • Polygraph Examinations No employee shall be compelled to submit to a polygraph examination against his/her will. No disciplinary action or other recrimination shall be taken against an employee refusing to submit to a polygraph examination, nor shall any comment be entered anywhere in the investigator’s notes or anywhere else that the employee refused to take a polygraph examination, nor shall any testimony or evidence be admissible at a subsequent hearing, trial, or proceeding, judicial or administrative, to the effect that the employee refused to take a polygraph examination.

  • Physical Exams The Employer agrees to provide without cost to employees, physical examinations and/or other appropriate tests when such tests are deemed necessary by management to determine whether the health of employees is being or has been adversely affected by exposure to potentially harmful physical agents, toxic materials, or infectious agents, or by attacks and assaults. The Employer agrees to provide to each affected employee who requests it a complete and accurate written report of any such medical examination or other appropriate tests related to occupational exposure. Additionally, written results of an industrial hygiene measurements or investigations related to an employee's occupational exposure will also be provided, upon request, to the employee or the employee's authorized representative. The Union and/or members of the applicable Health and Safety Committee will be provided copies of summary reports, but such reports will not contain personally identifying information.

  • Polygraph Examination No employee shall be compelled to submit to a polygraph examination. No disciplinary action whatsoever shall be taken against an employee refusing to submit to a polygraph examination; nor shall any comment be anywhere recorded indicating that an employee offered to take, took or refused to take a polygraph examination unless otherwise agreed to in writing by the parties; nor shall any testimony or evidence of any kind regarding an employee's offer to take, refusal to take, or the results of a polygraph examination be admissible in any proceeding pursuant to this Agreement, unless otherwise agreed to in writing by the parties.
