Enrollment Reports. 14.6.1 TDH will provide HMO enrollment reports listing all STAR Members who have enrolled in or were assigned to HMO during the initial enrollment period.

Enrollment Reports. The Contractor shall submit to the Division information respecting all new enrollments, disenrollments, reinstatements and circumstances affecting the enrollment status of Enrollees, as received by the Contractor, in a submission format approved by the Division. The Contractor shall review each Enrollee Listing Report upon receipt and shall submit all corrections to the Division on or before the fifteenth (15th) day of the month for which the Enrollee Listing Report is issued. Adjustments will be made to the next Enrollee Listing Report to reflect corrections, and the enrollment or disenrollment of Enrollees reported to the Division (and approved by the Division in the case of voluntary or involuntary disenrollment for cause) on or before the fifteenth (15th) calendar day of each month.
Enrollment Reports. The District shall closely monitor the actual class sizes throughout the schools. A report shall be made by the Administration to the Association during the fall and spring semesters each year upon request by the Association. Each report shall provide data regarding enrollment, staffing allocation, and class size and shall include information regarding class counts. The actual class count for all regular classes in the high schools shall be made available at the time of these reports.
Enrollment Reports. (a) PHS and Guardian shall prepare periodic and timely reports as set forth in Schedule C showing all Managed Care Contracts sold, renewed or terminated, and other enrollment information required by either party in order to perform its duties with respect to such Contracts. Enrollment Reports. HN Life shall prepare and deliver to United (i) on the Effective Date and on or before the tenth Business Day of each calendar quarter after the Effective Date until the end of the Transition Period enrollment reports (each, an “Enrollment Report”) which list, as of the last day of the calendar month preceding each such Enrollment Report, the name, address, telephone number and renewal date of each HN Life Employer Group, the name, address and telephone number of each HN Life Employer Group plan administrator and Broker/Consultant, the names of Members, including any HN Life Employer Groups or Members added to the Membership as a result of retroactive adjustments, any off- cycle termination since the previous month’s report, and such other information as specified in Schedule 2.4(a)(i) and (ii) on the Effective Date and on a weekly basis thereafter (with the exact day of delivery to be agreed upon by the Parties), reports (each, an “Accumulator Report”) which list, as of the end of the week preceding each such Accumulator Report, accumulator data, as prepared in accordance with HN Life’s past practices, for each HN Life Employer Group and Member, including such other information as specified in Schedule 2.4(a)(ii). Each Accumulator Report shall be provided in a consistent electronic format. Any updates to a previously delivered Enrollment Report resulting from retroactive adjustments to Membership shall be reflected on the next applicable Enrollment Report following the availability of such data. For the avoidance of doubt, HN Life’s obligation to deliver Enrollment Reports to United shall be in addition to Seller’s obligation to deliver the Initial Membership Statement, the Effective Date Membership Statement and updates thereto pursuant to the terms of the Stock Purchase Agreement.
Enrollment Reports. Not later than the date established by the Tennessee State Department of Education (the “Department”) for determining average daily membership, the Sponsor shall submit to the Department, with a copy to the Chartering Authority, the following information: The number of students enrolled in the Charter School. An Annual Enrollment Report pursuant to TCA 49-13-106(a)(3)is due by September 30th of each year including at least the following information: ▪ List of students who applied, date of application, ▪ All students accepted and priority category of each accepted student, ▪ Number, names, and priority category of admitted students, ▪ Number, names, and priority category of enrolled students, ▪ Number and names remaining on waitlist, ▪ Number of applications received after the lottery deadline ▪ Details of the School’s student recruiting policies. The Sponsor shall ensure that student enrollment count records are maintained electronically on a system that is mutually acceptable to the Sponsor and the Chartering Authority.

Enrollment Reports. Company will arrange to prepare and furnish to Group monthly enrollment reports of Members assigned to Group no later than the fifteenth (15th) day of each month. Company will exercise best efforts to provide quarterly utilization management reports.
Enrollment Reports. Within 10 days from the end of each quarter, the Borrower shall provide the Bondholder Representative with a copy (which may be by electronic transfer) of each report on enrollment by grade, headcount, membership, attendance and any other similar reports as requested, including enrollment statistics as of the first day of the Borrower’s then current school year, the October enrollment count of the Borrower’s then current school year, and the enrollment as of May 1 of the Borrower’s then current school year.
Enrollment Reports. The Site shall use its reasonable efforts to enroll as Study Subjects the required number of subjects meeting the requirements in the Protocol. Site shall obtain from Study Subjects properly executed informed consents in a form that CRO has approved, provided to the Site and has been approved by the State Institute for Drug Control. During the course of the Study, the Site shall advise CRO of the progress of the Study by written report as reasonably requested by CRO and by a complete and accurate written report for each Study Subject in a format specified in the Protocol (the “Case Report Form”).
