Equipment Safety Requirements. If BSLD and BellSouth mutually agree that the only location at the requested Central Office by which BSLD can obtain an unobstructed line-of-sight for the GPS signal is on the roof of the Central Office building, the roof-mounted GPS antenna and the associated GPS equipment located in BSLD’s physical collocation space must meet the following safety requirements: • Telcordia Network Equipment Building System (NEBS) Requirements, Criteria Xxxxx 0, as outlined in Telcordia Special Report SR-3580, Issue 1, • Bellsouth Engineering and Installation StandardsCentral Office Equipment, TR 73503-11, Section 6.14 • American National Standards Institute: - Telecommunications – Electrical Protection of Communications Towers and Associated Structures ANSI T1.334-2002 - Telecommunications – Electrical Protection of Telecommunications Central Offices and similar Type Facilities. ANSI T1.313-2003 2.4.1 The operation of BSLD’s GPS antenna and associated equipment shall comply with all applicable federal and state radio frequency guidelines.

Equipment Safety Requirements. CLEC’s Microwave Entrance Facilities and outdoor and indoor radio units, cabling, grounding equipment, antennas, masts, sled mounts and conduit must comply with all industry safety codes and the following specific safety requirements: • Telcordia Network Equipment Building System (NEBS) Requirements, Criteria Level 1, as outlined in Telcordia Special Report SR-3580, Issue 1FCC OET Bulletin 65, dated 08/97 • AT&T-13STATE Engineering and Installation StandardsAmerican National Standards Institute: • Telecommunications – Electrical Protection of Communications Towers and Associated Structures ANSI T1.334-2002 • Telecommunications – Electrical Protection of Telecommunications Central Offices and similar Type Facilities, ANSI T1.313-2003 • All federal, state, and local codes for the specific area. For example, national building codes such as the Uniform Building Code (UBC), Building Officials and Code Administration (BOCA), and the Southern Building Code Congress International (SBCCI), when adopted by the local municipality as the code of record for that area.
Equipment Safety Requirements. Any and all rented equipment shall comply with all current, applicable national, federal, state and local codes and regulations. Such standards include, but are not limited to, those promulgated by: U.S. Department of Labor OSHA U.S. Department of Transportation California Department of Industrial Relation CAL/OSHA American National Standards Institute Certain City Departments such as the Port of San Francisco and the San Francisco International Airport may be required to meet more stringent standards than those mentioned above when using equipment under this contract; in such cases the more stringent requirements shall apply and contractor will have the responsibility to deliver equipment that meet those standards. An example of this is the Airport requirement for certification of aerial basket trucks and the Port's requirements for certification of cranes. Upon request, Contractor shall supply copies of all applicable certifications that shall be valid for the term of the contract. Contractor shall provide copies of manufacturer's operating and maintenance procedures and instructions for each piece of equipment ordered, as requested and/or applicable. All equipment provided to the City by the Contractor during the term of this contract shall be not older than five (5) years from the date of manufacture.
Equipment Safety Requirements. All equipment must be equipped in accordance with State laws and shall be equipped with amber lens flasher visible for one (1) mile in a 360 degree pattern and six inch diameter hazard lights visible from the rear that operate independently of the brake lights. In addition arrow board (30" x 60") must be on all sweepers and be fully operational.

Otherwise, it is recommended that you ship all items within 7 days after receipt of payment. • You provide us with valid proof of shipment or delivery. • The payment must be marked “eligible” or “partially eligible” in the case of Unauthorized Transaction claims, or “eligible” in the case of Item Not Received claims, for PayPal’s Seller Protection program on the Transaction Details page. • In the case of an Unauthorized Transaction claim, you must provide valid proof of shipment or proof of delivery that demonstrates that the item was shipped or provided to the buyer no later than two days after PayPal notified you of the dispute or reversal. For example, if PayPal notifies you of an Unauthorized Transaction claim on September 1, the valid proof of shipment must indicate that the item was shipped to the buyer no later than September 3 to be eligible for PayPal’s Seller Protection program. PayPal determines, in its sole discretion, whether your claim is eligible for PayPal’s Seller Protection program. PayPal will make a decision, in its sole discretion, based on the eligibility requirements, any information or documentation provided during the resolution process, or any other information PayPal deems relevant and appropriate under the circumstances. To be eligible for PayPal’s Seller Protection program for a buyer’s Item Not Received claim, you must meet both the basic requirements and the additional requirements listed below: • Where a buyer files a chargeback with the issuer for a card-funded transaction, the payment must be marked “eligible” for PayPal’s Seller Protection on the Transaction Details page. • You must provide proof of delivery as described below.
