EXPRESS ASSUMPTION OF RISK AND RESPONSIBILITY. In recognition of the inherent risk of the travels and related activities in which you are intending to engage, you confirm that you are physically and mentally capable of participating in the Journey and that you willingly and voluntarily assume full responsibility for any injury, loss or damages suffered by you or caused by you. By registering for the Journey, you certify that you do not have any mental, physical or other condition or disability that would create a hazard for yourself or other Participants. Air Journey reserves the right in its sole discretion to accept, decline to accept or remove any Participant on the Journey and reserves the right, subject to the terms and conditions contained herein, to withdraw any part or all of the Journey and to make such changes as may be necessary.
EXPRESS ASSUMPTION OF RISK AND RESPONSIBILITY. In recognition of the dangers, hazards and risks (foreseen and unforeseen) associated with attending and participating in the OUTLIFE Outdoor Summer Camp, the undersigned confirms that the participant is physically and mentally capable of attendance and participation in all activities and use of all equipment associated with the Summer Camp. The participant is willingly and voluntarily attending and participating, and the Undersigned agrees that they and the participant shall assume all dangers, hazards and risks (foreseen and unforeseen) inherent in, arising from or related to the participant’s attendance and participation in the OUTLIFE Outdoor Summer Camp. Due to unavoidable circumstances (any natural calamity). Outlife or any other person associated with the same shall not be responsible for any claim or compensation PARTICIPANT RESPONSIBILITIES The Undersigned and the participant agree to the following: Acceptable Conduct by Participant. During attendance and participation in the Summer Camp, which may include travel to and from the campsite, the participant will act in a responsible manner and will abide by the instructions of any OUTLIFE personnel, and will comply and follow the rules and regulations of Outlife Adventure Travel Pvt. Ltd. In anticipation of the participant's enrolment in the OUTLIFE Summer Camp, the Undersigned and participant have consulted with a medical doctor with regard to the participant's medical condition. The participant has no physical or mental conditions which would cause him/her to be a danger to himself/herself or to others, is capable of Parent/Guardian's Signature: participating in all activities associated with the Summer Camp and has submitted the required and physician’s report.

In recognition of the inherent risks of the facilities on this property and the activity which I and any minor children to which I am responsible, will engage in, I confirm that I am (we are) physically and mentally capable of participating in the activity and using the facilities with the utmost of care. I understand that there is no life guard on duty and I am solely responsible for the safety and supervision of minor children, myself and my guests. I am totally aware of the risks, and expressly assume all of the risks and take full responsibility.

COVID-19 ACKNOWLEDGEMENT Guest must sign an acknowledgement attesting to their fitness to travel and agreeing to comply with all health protocols as directed by Air Journey staff during their journey. Guest may not travel if they test positive for COVID-19 during the 14 days prior to departure; standard cancellation terms will apply. Guest must maintain adequate physical distancing and frequently wash or sanitize hands during their journey. Masks are required in vehicles, indoor spaces, any outdoor spaces where physical distancing is not possible and in accordance with local regulations. Noncompliance with these measures will result in not being able to continue on the journey. Guest voluntarily assumes all risks and related expenses in the event that they or any member of their travelling party require testing, quarantine or become infected with COVID-19.
EXPRESS ASSUMPTION OF RISK AND RESPONSIBILITY. In recognition of the inherent risk of the travels and related activities in which you are intending to engage, you confirm that you are physically and mentally capable of participating in the activity, that you are willingly and knowingly electing to participate in this tour in spite of the potential risk of danger, and you willingly and voluntarily assume full responsibility for any injury, loss or damage suffered by you or caused by you, whether caused in whole or in part by the negligence, fault or wrongdoing, whether expected or not, of the owners, directors, agents, officers, employees, contractors, or subcontractors of ZE or of other participants. You understand and acknowledge that due to the remoteness of where we travel, emergency evacuation and/or search and rescue may be delayed or unavailable and that medical facilities and supplies may be limited and you acknowledge that it is your responsibility to assess the impact such limitations may have on any existing medical condition(s). You understand and acknowledge that ZE reserves the right to accept or reject any participant for any reason, and ZE or its guide has the right to disqualify you from any trip activity, if in ZE ‘s or such guide’s judgment, you are incapable of that activity and/or your continued participation in the tour will endanger yourself or the safety of the group. It is your responsibility and obligation to inform ZE, at the time your reservation is made, of any medical or physical disability or limitation that might disable you or render you unable to perform or safely complete the tour or any activity on the tour. You further acknowledge that you and your doctor are the best judge of your own conditions and limitations and that it is incumbent upon you to fully disclose the full extent of any such conditions or limitations to ZE.
EXPRESS ASSUMPTION OF RISK AND RESPONSIBILITY. In consideration of being able to participate in the Activities, I, for myself and any minor children for whom I am a parent, confirm that I and/or my minor child are physically and mentally capable of participating in full in the Activities. I and/or my minor child participate willingly and voluntarily in the Activities and voluntarily assume full responsibility for personal injury, accidents, or illness including death, as well as damage or loss of personal property. I am aware that there is a risk of negligence by CINNAMON RAINBOWS SURF CO., LLC (“Released Party”), and/or each of its officers, directors, agents, employees, agents, volunteers or assigns (collectively, “Agents”), including the failure by the Released Party and/or its Agents to safeguard or protect me/us from the risks, dangers, and hazards of the Activities. I on behalf of myself and/or my minor child freely accept and fully assume all risks, dangers and hazards associated with participating in the Activities and the possibility of loss, personal injury or death resulting therefrom.
EXPRESS ASSUMPTION OF RISK AND RESPONSIBILITY. I knowingly, voluntarily and freely assume all such risks, whether foreseen or unforeseen in participation in this Event, even if arising from the active negligence of the City of Agoura Hills or others, and assume full responsibility for my participation, including, without limitation, any minor children for which I am responsible, for bodily injuries, personal injuries, accidents, death, disease, illnesses, loss or damage of personal property and expenses thereof.

The participant is willingly and voluntarily attending and participating and the Undersigned agrees that they and the participant shall assume all dangers, hazards and risks (foreseen and unforeseen) inherent in, arising from or related to the participant’s attendance and participation in the Bentley Summer Camp.
