Family Dependent. To be eligible to enroll as a Family Dependent, an individual must be either: a) the spouse of a Subscriber; or b) an unmarried dependent child, whose age is less than the Maximum Age for dependent children as stated on the Schedule of Benefits. 6.2.1 A dependent child is defined as: a) a natural child, b) an adopted child, c) a natural child or an adopted child of the Subscriber or the Subscriber’s spouse, for whom the Subscriber is obligated to provide health care coverage through a court order or qualified medical support order, or d) any other child of whom the Subscriber or the Subscriber’s spouse is the Legal Guardian, custodial parent or Legal Custodian. The PPO may periodically require documentary proof of such dependency. Eligibility shall cease for a dependent child who (i) reaches the Maximum Age, as set forth in the Group Master Policy; (ii) becomes married; or (iii) obtains full-time employment (except for disabled dependent children and students). Coverage for a Family Dependent will become effective only if the Subscriber has Family Coverage.

Family Dependent. Any person (i) who meets all applicable eligibility requirements of section 2-A; (ii) who is enrolled hereunder; and (iii) for whom the prepayment required by section 4 has been received by Health Plan.

  • Dependent Child If dependent children are covered under separate plans of more than one person, whether a parent or guardian, benefits for the child will be determined in the following order: • the benefits of the plan covering the parent born earlier in the year will be determined before those of the parent whose birthday (month and day only) falls later in the year; • if both parents have the same birthday, the benefits of the plan that covered the parent longer are determined before those of the plan which covered the other parent for a shorter period of time; • if the other plan does not determine benefits according to the parents' birth dates, but by parents' gender instead, the other plan’s gender rule will determine the order of benefits.

  • Family Care Employees may use vacation leave for care of family members as required by the Family Care Act, WAC 296-130.

  • Family The District shall contribute no less than eighty percent (80%) of the total cost of the premium toward family coverage. The employee shall pay the difference between the District contribution and the total cost of the premium for family dental coverage.

  • Family Illness The start of a family leave for a serious health condition of a family member shall begin on the date requested by the employee or designated by Management.

  • Family Care Leave In accordance with RCW 49.12 and WAC 296-130, employees shall be allowed to use any or all of their choice of sick leave or other paid time off to care for a family member (as defined above) who has a serious health condition or an emergency condition. Employees shall not be disciplined or otherwise discriminated against because of their exercise of these rights.
