Forms and Cards. HSBC shall provide, and Merchant shall only use, the applications sales/credit slips [if applicable] and Cardholder Agreements as are provided by HSBC, and not use any materials provided by HSBC other than in connection with an application for a Card or a Card transaction. When provided by HSBC with a new edition of forms with the instruction to replace the previous editions with the new editions by a certain date, Merchant shall do so in accordance with the instructions. HSBC and Merchant shall agree upon the design of Cards and applications. HSBC shall be responsible for card production and issuance and will pay for the issuance of Cards to Cardholders. The terms and conditions set forth on the applications, Cardholder Agreements, and other forms shall be determined by HSBC and are subject to change by HSBC from time to time.

Forms and Cards. Household will at its expense provide to Merchants standard Sales Slips, Credit Slips and other forms from time to time for use by Merchants in the Program, which documents may be changed from time to time by Household, provided that Merchants may use their standard forms of Sales Slips and Credit Slips subject to the review and approval of such forms by Household. Merchants agree to pay for Card application/agreements. The design of Cards shall be agreed upon by Household and Merchants, and Cards shall be produced at Household's expense. The design and content of billing statements and the terms and conditions of Accounts and Card application/agreements shall be determined by Household and are subject to change by Household from time to time. The expense of changes to Card application/agreements shall be borne by the party for whose benefit the change is made, with the parties dividing evenly the expense of changes that benefit both Household and Merchants except that Merchants shall bear the expense of any changes to Card application/agreements required by any change in Applicable Law.
Forms and Cards. From and after the Conversion Date, Household will (at Household's expense) provide to Merchant standard applications, cardholder agreements, and other forms from time to time for use by Merchant in the Program, and will provide Cards to Cardholders, which documents may be changed from time to time by Household. Household and Merchant have mutually agreed on the form and content of Sales Slips and Credit Slips, which shall be printed by Merchant at Merchant's expense. Household shall, on a monthly basis, calculate the number of applications submitted to Household as compared to the number of applications that have been supplied to Merchant (other than the applications initially supplied to Merchant) and, if the number of applications supplied is likely to exceed the number of applications submitted to Merchant for such fiscal quarter by at least ten percent (10%), Household shall notify Merchant of such potential excess. The cost of the applications that have been supplied but not submitted in excess of ten percent (10%) shall be subject to reimbursement by Merchant at the end of such fiscal quarter. Household and Merchant shall agree upon the design of Cards and billing statements, which from and after the Conversion Date, shall be printed, manufactured and distributed by Household at Household's expense; provided, however, that statement inserts promoting Merchant's products will be included in such billing statements, shall be in a form and substance mutually agreed upon by the parties (other than the content of such statement inserts, which content shall be determined solely by Merchant) and Merchant shall be solely responsible for the production costs of such statement inserts. From and after the Conversion Date, Household shall maintain and issue the following Cards (in addition to other Cards agreed to by Household and Merchant): (i) Gottschalks Card; (ii) Gottschalks Gold; (iii) 55 Plus; and (iv) 55 Plus Gold (the "Current Cards"). No changes shall be made to the design or logo of the plastics of the Current Cards without the prior consent of Merchant. The terms and conditions set forth on the applications, agreements, and other forms shall be determined by Household, subject to Article 4 below, and are subject to change by Household from time to time.

  • UNIFORMS AND EQUIPMENT Section 1. Sworn employees of the Sheriff’s Office who are assigned to positions requiring a uniform shall receive Agency-provided uniforms and accessories in accordance with Agency policy at no cost to the employee. Uniforms worn out through normal use shall be replaced by the Sheriff’s Office as determined and approved by the supply staff. Section 2. Employees shall be permitted to purchase additional approved uniform pieces, excluding badges, from the Purchasing and Materials Division, provided sufficient quantities are on hand to cover the normal, required provision of uniforms to employees as per Section 1 above. Employees must pay for these items at the time of purchase or order. Section 3. Employees assigned to uniformed positions shall be eligible to receive a uniform maintenance allowance in the amount of $150 per year provided the employee spent at least 182 days of the calendar year in a uniform assignment. This allowance will be paid to all eligible employees in the last paycheck of the calendar year each December during the term of this Agreement. Section 4. The Sheriff’s Office shall issue body armor to employees and replace same in accordance with General Order 5-1. Section 5. In the event an employee leaves the employ of the Sheriff’s Office, he shall return to the Sheriff’s Office all equipment, uniforms and accessories, including those personally purchased by the employee in accordance with Section 2 above. Section 6. Employees whose prescription eyeglasses, contact lenses, hearing aids or watches are lost, damaged, or destroyed in the line of duty, except in cases where employee negligence caused or contributed to same, will be reimbursed for repair or replacement subject to the criteria detailed below. The Sheriff’s Office shall not reimburse for any other personal property lost, damaged or destroyed in the line of duty. The maximum reimbursement for prescription eyeglasses or hearing aids is the actual cost to repair or replace the item or $150, whichever is less. The maximum reimbursement for contact lenses or watches is the actual cost to repair or replace the item or $50, whichever is less. Requests for reimbursement for the personal property shall be made in writing to the employee’s immediate supervisor during the work shift in which the article of personal property was lost, damaged or destroyed. Except in cases of loss or when an item is repairable, the item for which reimbursement is sought should be turned in along with the written request for reimbursement. To aid in establishing the amount to be reimbursed, the employee will be required to provide to the Sheriff’s Office the receipt for the repair or replacement prior to reimbursement which must be approved by the Sheriff or his designee.

  • Identification Cards Identification (“ID”) cards are issued by Us for identification purposes only. Possession of any ID card confers no right to services or benefits under this Contract. To be entitled to such services or benefits, Your Premiums must be paid in full at the time that the services are sought to be received.

  • Cards 2.1 The Card is a prepaid payment card which may be used to pay for goods and services at participating retailers that accept prepaid Visa cards. The physical card may be used in shops and retail locations where the Cardholder is physically present or for online and other distance purchases. Your Card cannot be used to make cash withdrawals from ATMs and banks. Like any Card, We cannot guarantee that a particular retailer will accept the Card – Cardholders should check with the retailer before attempting the transaction if unsure. The Cardholder will not be able to use the Card to make any purchases from some retailers; such retailers have been blocked by Our systems in order to prevent the potential use of cards for unauthorised or unlawful activity. 2.2 The Card is an electronic money (“e-money”) product, regulated by the Financial Services Commission (“FSC”), Gibraltar. It is a prepaid card not a credit card and is not linked to Your bank account. You must ensure that You have sufficient Available Funds in the Master Wallet for the Cardholder to pay for each purchase or payment using the Card. The Card is intended for use as a means of payment, and funds loaded onto the Card do not constitute a deposit. You will not earn interest on the balance of the Card. The Card will expire on the Expiry Date and will cease to work. Please check clause 8 of this Agreement for further information. 2.3 The electronic money associated with the Card is issued by IDTFS to You. 2.4 When the Cardholder receives the Card, it will be issued in an inactive state. The Cardholder will need to activate it by logging onto Our Website, in Your personal login area (“Online Account Centre”) and then following the login and activation request for the Card prior to use. The Card will normally be ready for use one hour after activation. If the Cardholder does not activate the Card, any transactions that the Cardholder attempts to carry out may be declined.

  • Other Programs Nothing contained in this performance stock unit agreement shall affect the right of the Award Holder to participate in and receive benefits under and in accordance with the then current provisions of any pension, insurance, profit-sharing or other employee benefit plan or program of the Corporation or of any Subsidiary of the Corporation.

  • Graphics Landlord shall provide and install, at Tenant's cost, all letters or numerals on the exterior of the Premises; all such letters and numerals shall be in the standard graphics for the Building and no others shall be used or permitted on the Premises without Landlord's prior written consent.

  • Artwork Licensee must use the Java Logo(s) only in the exact form of approved camera-ready artwork or electronic artwork received from Oracle or Oracle's designee.

  • DRESS 6.1 Players shall be required to provide and wear their own playing shirts for the Event (“Event Shirts”); which must be worn throughout the Event and in any promotional work associated to the Event with dark smart trousers and plain black shoes/smart black sneakers. No waistcoats (vests), bow ties, denim jeans, t-shirts, jackets or sports shoes will be permitted. a) The Player will be entitled to wear a maximum of 4 (four) logos (for their own benefit and remuneration) on their Event Shirts ("Player Logos") throughout the Event. Player Logos must not exceed 7.5 cm x 5 cm (3” x 2”) or 6 square inches and shall not be permitted to be positioned on the left-chest of each Players Event Shirt or above or below the Event Logo. The Player shall submit to the Promoter no later than 1 (one) week before the first day of the Event details of the Player Logo’s for approval; b) Player logos shall include all logo’s, for the avoidance of doubt logos of federation’s and billiards clubs shall be included in the 4 logos permitted. Player country flags and names shall not be included as a logo; c) the Promoter shall have absolute discretion to disallow a Player the wearing of any Logo that, in the opinion of the Promoter, conflicts with the Event, Promoter, Sponsors, Broadcasters of the Event; d) the Promoter shall reserve the right, to be exercised in its sole discretion, to require the Player to display in addition to the above, a minimum of 1 (one) logo on the Event Shirts to be positioned as instructed by the Promoter for the sole use and exploitation by the Promoter (“Event Logo”); e) the purpose of this clause is not to act as an unfair restraint of trade or otherwise prevent the Player from earning a living.

  • Photographs You give your permission to us to use any photograph or photographic image including video or video stills taken of you while you are in any public spaces, grounds, offices at the Property or any sponsored events at the Property. You grant us and the Manager and our designees, the irrevocable and unrestricted right and permission to copyright, in its own name or otherwise, and to use, re-use, publish and re-publish photographic or video portraits or pictures of you or in which you may be included, without restriction as to changes or alterations. This usage may be in conjunction with your own name or a fictitious name. It may involve reproductions in color or otherwise that may be made through any medium, and in any and all media now or hereafter known. Usage may include illustration, promotion, art, editorial, advertising, trade, or any other legal purpose. You also consent to the use of any printed matter in conjunction with that usage. You waive any right that you may have to inspect or approve the finished product, the advertising copy or other matter that may be used in that regard. I hereby waive any right that I may have to inspect or approve the finished product and the advertising copy or other matter that may be used in connection therewith or the use to which it may be applied. You agree that we, the Manager and our designees will have no liability due to any blurring, distortion, alteration, optical illusion, or use in composite form that may occur in taking or usage of any picture or in the subsequent processing or publication of the picture. You release us, the Manager, and our designees from all claims of any nature arising in any way from the use of your photograph or photographic image. This release contains the entire agreement on this subject matter.

  • Manuals Each product delivered under contract to any Customer must be delivered with at least one (1) copy of a safety and operating manual and any other technical or maintenance manual. The cost of the manual(s) must be included in the price for the Product offered.
