Common use of Hospitalization, Surgical and Major Medical Insurance Clause in Contracts

Hospitalization, Surgical and Major Medical Insurance. Section 1 The Board will provide (Single and Family) health coverage comparable to or better than the MMO Super Med Select coverage in effect as of January 1, 2016 (include dependents through age 26), for participating bargaining unit members. Changes in the insurance coverage, to take effect July 1, 2016 include: • Co-insurance: 80% in network, with $500/1,000 out of pocket maximum: 60% out of network, with $1,000/2,000 out of pocket maximum. • Office visit co-pay; $20 primary; $20 specialist - - (routine office visits are covered at 100%, and are not subject to the deductible) $25 urgent care; and $50 emergency room. • Deductible: In-network deductible $350/700; out of network deductible $700/1,400.
